Failure Modes Effects Analysis case studies

A case study is carried out for a pig launcher that is widely used in oil and gas facilities during pigging operations. Failure Mode, Effects and Criticality Analysis (FMECA) approach is used to identify the potentially critical human actions/interventions based on guided brainstorming. After that, these identified actions/interventions analyzed using conventional FMECA approach and MEDIA. FMECA can provides a qualitative results, while MEDIA can provides quantitative results and also demonstrates the influence of an organization on human reliability. [Pg.997]

A mixture of qualitative and quantitative methods was used in this work. These include reliability analysis, eapital investment costs, safety, feasibility and environmental impaet assessment (ElA). The Gulf of Mexico has between 5000 and 6000 platforms—many of them small and in shallow water. It is simply not economically feasible to write a safety case for each platform. The system safety case of corrrse varies from sector to sector. The core of a nuclear system safety case is (i) a deterministic analysis of the hazards and farrlts which could arise and cause injury, disability or loss of life fiom the plarrt either on or off the site, and (ii) a demonstration of the sufficiency and adequacy of the provisions (engineering and procedural) for ensuring that the combined frequencies of such events will be acceptably low. Safety systems will feature amongst the risk reducing provisions comprised in this demonstration, which will thus include qualitative substantiations of compliance with appropriate safety engineering standards supplemented (where practicable) by probabihstic analyses of their reliabilities. In this paper, we propose and investigate formal concepts that aim to overcome this bias. They support the construction of FMEA tables solely based on the system model and the failure modes, i.e., without requiring the set of effects as input. More concretely, given a system specification in the Architecture Analysis and Design Language (AADL), we show how to derive relations that characterize the effects of failures based on the state transition system of that specification.
