This section s purpose is accomplished by discussing one failure and mentioning a few others with the hope that the reader will continue to be a student of failures. An excellent resource is Norbert J. Delatte, Jr s book Beyond Failure Forensic Case Studies for Civil Engineers (2009), which uses a case study approach to draw technical, procedural, and ethical lessons from many unfortunate failures. The over 40 case studies are organized by specialty areas such as statics and dynamics structural analysis and fluid mechanics and hydraulics and management, ethics, and professional issues. 

This case study concerns the initial design and redesign of a security cover assembly for a solenoid. The analysis only focuses on those critical aspects of the assembly of the product that must be addressed to meet the requirement that the electronics inside the unit are sealed from the outside environment. An FMEA Severity Rating (S) for the assembly was determined as S = 5, a warranty return if failure is experienced.

Although meta-analysis of studies in acute myocardial infarction suggested that nitrates protect against death and reinfarction (88), the ISIS-4 study found no such protection for 1 month after infarction (89). However, this study did show that nitrates are relatively safe in suspected myocardial infarction, even though 14% of patients who received nitrates had heart failure. Compared with placebo-treated patients there was an excess of hypotension considered severe enough to withdraw the drug in 15 per 1000 patients in about half the cases it occurred soon after the start of treatment. The excess risk for headache was 18.7 per 1000 and of dizziness 1.6 per 1000, 0.8 per 1000 being associated with profound hypotension.

In this chapter, we will present case studies where docking and HTD methods accounting for protein flexibility were used in the context of drug discovery/design or lead optimization projects. This vdll be preceded by a theoretical analysis of the problem, followed by concrete examples of docking failures stemming from neglecting protein flexibility. 

In the next two chapters we will discuss some case studies regarding actual failures and reference will be made to the above discussion. A formal analysis of the results will then be presented in Chapter 10 with a general discussion of the implications of the conclusions. 

It is very clear from the complexity of the situations described in the case studies of the last two chapters, that simple factors of safety, load factors, partial factors or even notional probabilities of failure can cover only a small part of a total description of the safety of a structure. In this chapter we will try to draw some general conclusions from the incidents described as well as others not discussed in any detail in this book. The conclusions will be based upon the general classification of types of failure presented in Section 7.2. Subjective assessments of the truth and importance of the checklist of parameter statements within that classification are analysed using a simple numerical scale and also using fuzzy set theory. This leads us on to a tentative method for the analysis of the safety of a structure yet to be built. The method,however, has several disadvantages which can be overcome by the use of a model based on fuzzy logic. At the end of the chapte(, the discussion of the various possible measures of uncertainty is completed. 

This thesis aims at offering a reference for overall risk management of deep foundation pit construction, in addition, collecting basic data for risk theoretical analysis and study. Furthermore it is more important to learn a lesson from failure cases and accumulate experience in order to provide reference for the subsequent deep foundation pit construction.
