However, the establishment of a new endotoxin standard by the World Health Organization is a recent example of successful international collaboration between the World Health Organization, the United Stated Pharmacopoeia and the European Pharmacopoeia (Poole et al. 1997). Thus this standard is available from any of these organizations to be employed as a reference in the harmonized Limulus Amoebocyte Lysate test. 

In many instances the influence of pyrogens on body temperature is indirect. For example, entry of endotoxin into the bloodstream stimulates the production of IL-1 (Chapter 9) by macrophages. It is the IL-1 that directly initiates the fever response (hence its alternative name, endogenous pyrogen ). 

A variety of lipid adjuvants and protein mediators have also been shown to influence the immune response to antigens encapsulated in liposomes. The most widely used examples of such adjuvants for practical immunization procedures are endotoxin (including lipid A and lipopolysaccharide) and numerous types of lipophilic derivatives of muramyl dipeptide. 

Next was the observation that histamine is liberated in tissues by an antigen-antibody reaction and is responsible for a large share of the symptoms of anaphylaxis and the allergic state. Histamine plays an important role in physiological functions of animals and man for example, histamine levels change in certain pathological conditions. Histamine levels or its formation are affected also by various stress situations, such as exposure to chemical compounds, endotoxins, hyper- or hypothermic conditions. 

The opposite of harmony is disharmony. An example of disharmony is the need to repeat tests using rabbits for pyrogen where testing for bacterial endotoxin is otherwise prescribed. This represents the most extreme disharmony of methods. But there is even a greater disharmony, and that would be to reach different eonelusions as to pass/fail the speeimen. In this ease, the quality control professionals must make a judgment as to whether or not this material can be sold in one or more regions. Functionally equivalent to harmonization is the absence of disharmony. Because of a difference in policy, pharmacopoeias may not differ on adoption of a test at all. If certain tests are eonsidered neeessary by one pharmacopeia in order to protect the consumer, it is appropriate for that pharmacopeia to adopt the test without reference to any other region. 

On completion of intermediate purification, most contaminants are removed. For example, endotoxin concentrations are reduced by more than two orders of magnitude, while the specific activity of recombinant protein, a measure of purity, is increased several-fold. Some observations have suggested that presence of microbial compounds in the environment may affect hosts immune response. For example, the children of farmers who are exposed to high levels of endotoxins exhibit increased expression of TLR2 and CD14 compared to children with less exposure. This suggests a modulation of innate immune response in the absence of an infection. How this observation corresponds to a lower incidence in allergic disease needs to be determined. Activation of TLR induces inflammatory and acquired immune response, and inhibition of one of these pathways (MyD88) results in decreased THi responses and enhanced IgE production. 

The absorption of drugs in solution from intramuscular and subcutaneous sites of injection is limited by the perfusion rate. Failure to recognize this important concept has resulted in patient death. For example, morphine sulfate is often administered subcutaneously in a dose of 10 mg per 70 kg of body weight. This dose is sufficient to produce analgesia in 70% of patients with moderate to severe pain. However, in the setting of circulatory collapse and shock (e.g., septic shock in bacteremia due to release of endotoxin) in which the peripheral circulation may be impaired, morphine is not absorbed. Cases have been reported in which the lack of analgesia prompted the additional injection of morphine, all of which remained at the injection site and in the subcutaneous capillary bed. When the peripheral circulation improved, the massive amount of morphine that had collected became absorbed and death ensued, which was primarily due to respiratory depression. 

It is desirable to collect as many different matrices from each study participant as is feasible and to process them with consideration of both immediately planned analyses of biomarkers and future uses. For example, several Children s Environmental Health Centers obtained urine, peripheral blood, cord blood, breast milk, meconium, saliva, hair, placental tissue, infant formula, indoor and outdoor air, and house dust from longitudinal birth cohort studies (Eskenazi et al. 2005). The centers have analyzed concentrations of numerous compounds in those biologic and environmental samples, such as pesticides, phthalates, mercury, lead, cotinine, polycyclic aromatic hydrocarbone (PAHs), PAH-DNA adducts, allergens, endotoxin, antioxidant micronutrients, cholinesterase, and thyroid hormones. Most centers also banked samples for future analyses. 

High-affinity resins are perhaps most useful in removing problematic contaminants from the product stream. An example is the use of a column of polymyxin B (theoretical capacity of 6 mg/cc) to remove 90-98% of the residual endotoxin [148] from protein preparations.
