In order to promote safety, companies have contests, conduct periodic safety meetings, give prizes, award plaques, and conduct big advertising campaigns. Managers feel that making the employee aware of safety will help improve the company s over-all performance. Experience has shown them to be correct. 

A program is necessary for identifying all materials in the workplace, and making employees aware of the hazards of these materials and the necessary precautions to be taken to prevent or control personnel exposure. Materials Hazard Identification and information gathering is an essential element of fire prevention. The hazardous properties of all chemical substances used in the workplace should be known in order to develop the appropriate design, routine handling practices, and fire prevention plan. 

A hasty modification can result in an accident, as discussed in Chapter 6. To address the problems of one-minute modifications, chemical plant management must persistently encourage employee awareness and train their employees about the potential dangers that can be created by the quick, inexpensive substitutions. It is essential that well-maintained engineering and equipment specifications are readily available. Changes which might include improper substitutes (such as incompatible materials of construction) or improper procedures should be reviewed by a third party. However, this is sometimes easier said than done in the sometimes hectic pace of maintaining maintenance and production schedules. 

Each plant must persistently train and create employee awareness of potential dangers faced with even small changes that can be easily introduced into the plant. This message must be given not only to the engineers and supervisors, but also to the chemical process operators and mechanics.

The OSHA Log of Work-Related Injuries and Illnesses (Form 300) is used to document and classify work-related injuries and illnesses. The log also documents the extent and severity of each case. Employers use the log to record specific details about what happened and how it happened. The summary (Form 300A) shows totals for the year in each category. At the end of the year, post the summary in a visible location to make employees aware of the injuries and illnesses occurring in the workplace. Employers must keep a log for each establishment or site. If an employer operates more than one establishment, each location must keep a separate log and summary. Keep all logs for 5 years. 

System safety is an important part of the product life cycle. Allowing the individual employee to find the problems associated with work is no longer acceptable. A proactive approach is necessary because of increased product-liability litigation and increased employee awareness of process-safety concerns. Therefore, the safety professional must make every effort to understand the concepts of system safety.
