Natural course of the disorder. Scone apparent placebo effects may be due to spontaneous remissions whereas others are clearl not. Spontaneous remissions are only partly predictable for a given time interval. Patients on waiting lists show clearly the natural course of the disease, spontaneous remission and worsening (see Fig. 5.3). 

Initial treatment consists of NSAIDs, low dose corticosteroids. If the symptoms are not controlled then second line/disease modifying drugs (DMDs) are added. Since it is a progressive disease, spontaneous remissions are rare and joint damage occurs early, DMDs are started early and continued indefinitely with regular monitoring. 

Low-dose unfractionated heparin or LMW heparin can be used to prevent venous thromboembolism in other high-risk patients, e.g. those confined to bed and immobilised with strokes, cardiac failure or malignant disease. Spontaneous bleeding has not been a problem with this form of anticoagulant treatment. 

Clearly, coronary microembolization is a frequent event in ischemic heart disease, spontaneously in patients with unstable angina / acute coronary syndromes as well as artificially during coronary interventions with typical consequences, such as malignant arrhythmias and contractile dysfunction (Figure 7). Trichloroethylene (TCE) is the most common and abundant pollutant in ground-water in the United States. It is primarily used as a solvent to remove grease fiom metal parts, as a solvent for extraction of waxes, oil, fats, tar, and in several consumer products such as paints, carpet cleaning fluid, etc. It is estimated that between 9% and 34% of drinking water supply sources are contaminated with TCE. Several epidemiological studies link TCE exposure to health problems related to congenital heart disease, spontaneous abortion, cancer, etc.

The clinical picture is often variable (variable expressivity) and sometimes not evident at all (incomplete penetrance). NIDDM is a much more common disease than IDDM, accounting for about 85—90% of all cases of diabetes meUitus. Whereas NIDDM may be present at any age, the incidence increases dramatically with advanced age over 10% of the population reaching 70 years of age has NIDDM. Patients with NIDDM do not require insulin treatment to maintain life or prevent the spontaneous occurrence of diabetic ketoacidosis. Therefore, NIDDM is frequendy asymptomatic and unrecognized, and diagnosis requires screening for elevations in blood or urinary sugar. Most forms of NIDDM are associated with a family history of the disease, and NIDDM is commonly associated with and exacerbated by obesity. The causes of NIDDM are not well understood and there may be many molecular defects which lead to NIDDM. 

Asthma is an extremely complex condition characterized by variable and reversible airways obstmction combiaed with nonspecific bronchial hypersensitivity (1 3). The cause of asthma, which is not always readily diagnosed (4), remains unknown. Days, if not weeks, ate needed to document the spontaneous reversal of the airways obstmction ia some patients. Asthmatics experience both an immediate hypersensitivity response and a delayed late-phase reaction, each mediated by a different pathway. Chronic asthma has come to be viewed as an inflammatory disease (5). The late-phase reaction plays a key role ia iaduciag and maintaining the inflammatory state which ia turn is thought to iaduce the bronchial hyperresponsiveness (6). The airways obstmction results from both contraction of airways smooth muscle and excessive bronchial edema. Edema, a characteristic of inflammatory states, is accompanied, ia this case, by the formation of a viscous mucus which can completely block the small airways. 

In certain diseases, such as osteomalacia, syphilis, and osteomyeHtis, bones break spontaneously and without a trauma. The severity of the fracture usually depends on the force that caused the fracture. If a bone s breaking point was exceeded only slightly, then the bone may crack rather than break all the way through. If the force is extreme, such as in an automobile collision or a gunshot, the bone may shatter. An open or compound fracture is particularly serious because infection is possible in both the wound and the bone. A serious bone infection can result in amputation. 

Transgenic animal models with spontaneous or induced receptor gene defects have been instrumental in elucidating the physiological roles of the LDL receptor gene family. In addition, a number of human diseases have been identified that are caused by sporadic or inherited forms of receptor deficiency (Table 1).
