Anti-ribosomal antibiotics are enzyme inhibitors, and the ones that are dinically useful inhibit bacterial ribosomes far more effectively than they inhibit eukaryotic ribosomes. Many enzyme inhibitors exert their effects by binding to the active site of enzymes and thereby prevent the binding of substrates. Others block enzyme function by inhibiting conformational changes essential for enzyme activity. Anti-ribosomal antibiotics are unexceptional in this regard. They bind to the sites on both subunits to which tRNA also binds. Some block the interactions of substrates with the ribosome. Others block the tunnel and thereby prevent the nascent peptide from extending.

Although dinical microbiology laboratories report microbial susceptibility test results as sensitive or resistant to a particular antibiotic, this is not an absolute predictor of clinical response. In a given patient s infection, variables such as absorption of the drug, its penetration to the site of infection, and its activity once there (influenced, for example, by protein binding, pH, concentration of oxygen, metabolic state of the pathogen, intracellular location and concentration of microbes) profoundly alter the likelihood that effective therapy will result. 

The co-administration of 5-flucytosine with AmB, which is commonly used dinically to obtain a synergistic antifungal effect, protects against acute and chronic nephrotoxicity [107]. The mechanisms by which flucytosine influences the renal response to AmB are not clear but may relate to (i) its administration in 0.9% NaCl, which itself is protective, (ii) a renal vasodilator effect of flucytosine that antagonizes AmB-induced vasoconstriction, and (iii) reduction in renal uptake of AmB [107].

Levels of IL-2 comparable to those used in human studies, caused vacuolization, cell shrinkage and growth inhibition. Dexamethasone, which is used dinically to inhibit TNFa, failed to protect the cultured cells from the effects of IL-2. TNFa when given alone had no apparent effect on morphology or cell growth, suggesting that the nephrotoxic effect of IL-2 was direct.

Resistance to Tetracyclines. The tetracyclines still provide inexpensive and effective treatment for several microbial infections, but the emergence of acquired resistance to this dass of antibiotic has limited their clinical usefulness. Studies to define the molecular basis of resistance are underway so that derivatives having improved antibacterial spectra and less susceptibility to bacterial resistance may be developed. Tetracyclines are antibiotics of choice for relatively few human infections encountered in daily clinical practice (104), largely as a result of the emergence of acquired tetracycline-resistance among dinically important bacteria (88,105,106). Acquired resistance occurs when resistant strains emerge from previously sensitive bacterial populations by acquisition of resistance genes which usually reside in plasmids and/or transposons (88,106,107). Furthermore, resistance determinants contained in transposons spread to, and become established in, diverse bacterial species (106).

The maaolides (e.g., erythromydn, clarithromydn, and azithromycin) are translocation inhibitors. Their spectrums of activity, dinical uses, biodisposition, and side effects are considered. Qindamydn is not a macrolide but shares the same mechanism of action. 

The citrate salt of isoaminile [77-51-0] (50) is a nitrile used as an antitussive in numerous European countries. In dinical trials it was shown to be as effective as codeine or chlophedianol, with few mild side effects. Isoaminile citrate is longer acting than chlophedianol and does not cause the respiratory depression of codeine (68). 

Buprenorphine, administered in methadone dinics, has both agonist and antagonist effects. In Florida, buprenorphine caused 312 deaths in the first six months of 2006, more than any other drug. 

The final step in drug development inevitably involves dinical trials, with all their associated expense, logistical difficulties, risk and ethical problems. It is to be hoped that the power-fid tools that are now at our disposal will result in a new generation of safer and more effective treatments in the near future. 

Because we focus on the dinical presentation of the artifacts, we will not work out the physical-technical cause of the artifact, nor discuss the best solution(s). The aim of this chapter is to illustrate the effect or the appearance of artifacts on the clinical image. 

Hemocompatibility tests evaluate effects on blood and/or blood components by blood-contacting medical devices or materials. In vivo hemocompatibility tests are usually designed to simulate the geometry, contact conditions, and flow dynamics of the device or material in its dinical application. From the ISO standards perspective, five test categories are indicated for hemocompatibility evaluation thrombosis, coagulation, platelets, hematology, and immimology (complement and leukocytes) (Table 12).
