Where pain is worsened by muscle spasm, oral muscle relaxants may serve as a useful adjunct to therapy.25 These agents include baclofen, metaxalone, methocarbamol, carisoprodol, and cyclobenzaprine. Muscle relaxants may decrease spasm and stiffness associated with either acute or chronic musculoskeletal disorders. These agents should be used with caution because they all may cause sedation, especially in combination with alcohol or narcotic analgesics.

Oral baclofen has also been used to reduce alcohol consumption in people who are chronic alcohol abusers.21,22 Apparently, relatively low doses of baclofen can reduce the cravings and desire for alcohol consumption via the effects of this drug on CNS GABA receptors.21 Future studies will help clarify the role of this drug in treating chronic alcoholism.

Oral baclofen has been studied in several other medical conditions. Preliminary studies suggest that it may be effective in reducing craving in recovering alcoholics. It has also been found effective in preventing migraine attacks in some patients.

The client should be warned not to consume any alcohol while taking baclofen. Baclofen is a central nervous system depressant, as is alcohol. The combination of alcohol and baclofen could intensify the depressant effects. 

The manufacturer of baclofen warns that baclofen may enhance the sedative effect of alcohol. However, tolerance to baclofen s sedative effect has been reported in alcohol-addicted patients after a period of abstinence, as well as after a relapse. ...

Sioufi ei al. made isobutyl esters of baclofen enantiomers by treating the acid from plasma or urine eluted from a solid-phase (Cig) column, with 0.5 ml of HCl in isobutyl alcohol made by adding 250 y of acetyl chloride to 5 ml of the alcohol. The reaction was effected at 100 °C for 15 minutes, after which the excess alcoholic HCl was evaporated to dryness in a stream of nitrogen [30]. 

Using chiral thiols for derivative formation, isoindoles are formed from amino acids and amino alcohols. BOC-L-cysteine, N-acetyl-L-cysteine, Af-acetyl-D-penicillamine [340] and l-thio-jS-D-glucose [341] have been shown to be suitable reagents which allow the separation of most amino acid enantiomers using reversed phase column chromatography. Thus, OPA/N-acetyl-L-cysteine has been used among others for the separation of enantiomers of aspartate [342], baclofen [343], norepinephrine, dopa [344] and lombricine [345]. 

The use of baclofen has been reviewed [33 ] and specifically in the treatment of pediatric spasticity [34 ], in adults with cerebral palsy [35 ], and in the treatment of alcohol dependence [36 ]. 

In a double-blind, placebo-controlled, randomized study of baclofen 30 mg/day over 12 weeks in 80 subjects with alcohol dependence, 66% completed the study [43 ]. Baclofen was well tolerated, and only two individuals withdrew because of adverse events.
