Sample preparation Condition a 3 mL 500 mg Megabond MF CIS SPE cartridge (Varian) with 3 mL MeOH and 3 mL water. Sonicate 1 g cosmetic with 10 mL MeOH or MeOH dichloromethane 10 90 (depending on what appears visually to give the best solubility) at 40° for 10 min, centrifuge, collect the clear supernatant. Add 5 mL of the supernatant to the SPE cartridge, wash with 4 mL acetonerwater 20 80, wash with 1 mL n-hexane, elute with 4 mL diethyl ether. Evaporate the eluate to dryness under reduced pressure, reconstitute the residue with 5 mL (or more) MeOH, inject a 10 aL aliquot. 

Mobile phase Isocratic.MeCN water 60 40. Gradient. MeCN water from 25 75 to 90 10 over 30 min, maintain at 90 10 for 10 min.

Sample preparation Mix 1 g of a topical product, 5 ml. 100 mM pH 4 citrate buffer saturated with NaCl, and 10 mL ethyl acetate, shake vigorously by hand to ensure that no large clumps stick to the tube, mix with the tube on its side on an oscillating shaker for 15 min. Remove the upper ethyl acetate layer and wash with citrate buffer as before. Dry the ethyl acetate over anhydrous sodium sulfate and evaporate to dr5mess. Dissolve the residue in a mixture of 10 mL heptane and 5 mL MeCNiwater 90 10. Remove the upper heptane layer and extract it with 2 mL MeCNiwater 90 10. Combine the MeCN/water layers and evaporate them to dryness, dissolve the residue in 300 jiL MeOH, filter (0.45 ixm nylon), inject a 5 aL aliquot.

Compounds having the 16,17 ketal, eg, budesonide, amcinonide, fluocinonide, halcinonide, triamcinolone acetonide, and flurandrenohde, also undergo metabohsm by routes that parahel that of cortisol metabohsm. Unsymmetrical acetals such as budesonide are also metabolized by routes not available to the more metabohcahy stable symmetrical 16a,17a-isopropyhdiene-dioxysubstituted compounds (desonide, flunisohde, and triamcinolone acetonide). Isozymes within the cytochrome P450 3A subfamily are thought to catalyze the metabohsm of budesonide, resulting in formation of 16a-hydroxyprednisolone and 6P-hydroxybudesonide (19,20) (Fig. 3) in addition to the more common metabohc steps (oxidation via reduction of A, etc).

Amcinonide Cyclocort cream, lotion, oint 0.1% High... 

Examples of group I, i.e. weak or low efficacy topical steroids, are hydrocortisone acetate in various concentrations, methylprednisolone 1.0% and prednisolone 0.5%. Group II, the moderately potent steroids, includes alclometasone dipropionate 0.05%, hydrocortisone butyrate 0.1%, triamcinolone acetonide 0.025% and fluocinolone ace-tonide 0.01%. Group III, the potent steroids, contains among others betamethasone valerate 0.1%, betamethasone dipropionate 0.05%, budesonide 0.025%, desoximetasone 0.05%, fluticasone propionate 0.05%, amcinonide 0.1%, fluocinonide 0.05% and mometasone furoate 0.1%. Group IV comprises the very potent agents such as clobetasol propionate 0.05% and halobetasol propionate 0.05%. 

Contraindications History of hypersensitivity to amcinonide or other corticosteroids. 

Potent, fluorinated topical corticosteroid with comparable efficacytofluocinonide, diflorasone, amcinonide, betamethasone, dipropionate, and halcinonide cost should govern use...
