Safety management checklist

Included in OSH As JHA Booklet, 3071, is a good description of a process hazard analysis (PHA) [1]. This is being used in the Process Safety Management (PSM) program (29 CER 1910.119) to understand how hazards exist. There are some good methods listed in the manual that can be used to conduct a JHA. As you review each method you can determine which one may be useful for your operation. The typical method chosen is the checklist. 

Wlien a contractor/subcontractor is hired to perform work in a potentially hazardous area on one of your facilities or that of a client, the project manager shall provide the contractor/subcontractor a copy of the contractor site safety rules checklist for completion. The form and any other rules specific to that site must be signed, dated, and returned prior to any work being performed at a particular site. An executed copy will be made a part of the project file. 

This optional attachment is used to promote continuous improvement of the entire safety management system. It is a checklist for anyone to use as a guide when auditing incident investigation activity. The facility from which this example was derived includes a similar checklist for each safe work practice at the site. 

A one-page memory-jogging safety assessment checklist was made available by Trevor Kletz in 1976. [6] The checklist is still valid today. After a review of several Management of Change policies from several major companies, it appears that Kletzs checklist or a similar checklist was used as a basis for a few companies procedures. 

The site-level health and safety manager was given notice from corporate for the need to conduct the qualitative risk analysis of the plant s hazardous material transportation operations. The safety manager was provided with a questiotmaire regarding all chemicals of concern, shipping quantities, modes of transit, and the route characteristics (similar to the checklist found in Table 4.2). This information was transmitted back to corporate for analysis, which included a corporate-level comparison to the other XYZ Chemical facihties risk results. Based on the information collected at the site level, the following was determined and reviewed by corporate for this facility ...

Well, there are certainly no shortages of suggestions of how to do this, and what a safety management system should contain in order to achieve it. Many textbooks and consultancies can provide checklists and pro-formas to create a coherent safety management system, yet such a formulaic approach can be seen as a little incoherent for the management of something that doesn t actually happen, within the context of a messy, inconsistent, and constantly changing social reality. 

The list is meant only to be a starting point for those estabUshing a comprehensive safety control structure and a checklist for those who already have sophisticated safety management systems. It should be supplemented using other sources and experiences.

It is recommended that the reader compiles their own CM A checklist tailored to the circumstances and that such a checklist be maintained as a live document within the procedures of the Safety Management System. 

The lead auditor should prepare a list of questions pertinent to the scope of the audit. ISA-TR84.00.04-1 Annex D.4 Functional Safety Assessment provides example checklists that could be addressed during the audit. In addition, refer to the COPS book, Guidelines for Auditing Process Safety Management Systems," 2011. 

Transition Gate 3 checklist (transition complete review). This is a template agenda for the transition complete review meeting, which is called to confirm that all of the required transition activities have been completed and the service can progress to business as usual. It also details all of the deliverables required. The changes made were to clarify that a safety management plan, hazard log and safety justification report are required deliverables. 

Lack of initial involvement Although the safety behaviour checklists were circulated to managers for comment, it was only some time later that it was pointed out that the inclusion of mandatory items, such as the removal of all jewellery, caused problems. Non-compliance with these well-established items was a disciplinary offence. Inclusion of behavioural items of this type on a checklist conflicted with the no discipline promise given with the checklist. Such items were subsequently removed.
