Thyrotropin is an anterior pituitary hormone that stimulates the thyroid to produce and synthesize thyroxine (T4), triiodothyronine (T3), and thyroglobulin. Thyrotropin alpha is a commercially available analog of TSH that is used to help detection of metastatic differentiated thyroid carcinoma it is also known as recombinant human TSH (rhTSH). 

Alternatively, administration of recombinant human TSH (Thyrogen) can produce comparable TSH elevations without discontinuing thyroxine and avoiding hypothyroidism. Recombinant human TSH is administered intramuscularly once daily for 2 days. A rise in serum thyroglobulin or a positive 13 scan will indicate a recurrence of the thyroid cancer.

FSH is a member of a superfamily of proteins, all structurally related. which includes luteinizing hormone (LH), chori->inlc gonadotropin, and thyroid-stimulating hormone (TSH). It is u heterodimer, the a subunit contains 92 amino acids, and the subunit contains 111 amino acids. The protein IS rather heavily glycosylated and has a molecular mass of approximately 33 kDa. The traditional source for isolation t l FSH was postmenopausal urine, which provided a preparation that was less than 3% pure and wa.s signiricantly contaminated by LH. The recombinant human FSH (rhFSH) is produced in a mammalian cell line, the CHO. 

Immunometric assays for TSH are available commercially as manual kit procedures or for use on automated systems. For practical reasons, nonisotopic methods dominate the market and have replaced radioactive tracer methods in most routine laboratories. The majority of immunometric TSH assays label the detection antibody with chemiluminescent labelled molecules other labels include peroxidase or alkaline phosphatase and sensitive photo-metric and fluorescenri molecules. Other assays are based on the use of fluorescent labels using europium chelates chemiluminescent compounds such as acri-dinium esters or ruthenium or bioluminescent molecules such as recombinant aequorin. ... 

The a subunits of all pituitary glycoprotein hormones are identical but show minor differences from the a subunit of hCG. The subunits differ in all and confer hormonal specificity. The association of a and subunits is required for activity, but the subunits need not be homologous. Dissociation of the subunits of one hormone, followed by recombination with subunits of another hormone, results in hybrids that are biologically active and exhibit the biological activity characteristic of the y3 subunit. A considerable advance in the management of thyroid cancer has resulted from the introduction of recombinant human TSH (thyrogen). A dose of 0.9 mg is administered intramuscularly, followed by an identical dose 24 hours later. The serum thyroglobuLin is then measured 48-72 hours after the second TSH injection this has become the preferred diagnostic test for following patients with differentiated thyroid cancer who have had thyroidectomy and ablation of any remnant tissue with radioactive iodine. This regimen can also be used to stimulate iodine uptake by malignant cells in a total body scan (see below). 

In these patients, radioiodine is generally less favored and considered with reluctance, but should not be ruled out entirely, particularly since therapeutic uptake of radioactivity may be increased by stimulation with recombinant human TSH. Radioiodine may be a means to achieve some symptomatic refief in selected patients.

Iodide uptake after stimulation with TSH is sufficient in most differentiated thyroid cancers to utilize (3-emit-ting radioactive iodine for the treatment of residual and metastatic disease.
