Prurigo

TRPVl results in premature hair follicle regression and apoptosis [153]. This finding confers a therapeutic potential for TRPVl antagonists in the treatment of alopecia. In prurigo nodularis, enhanced TRPVl expression was found that normalized during capsaicin treatment [154].

Adrenal steroids of highest potency are reserved for recalcitrant dermatoses, e.g. lichen simplex, lichen planus, nodular prurigo and discoid lupus erythematosus. 

A 70-year-old woman treated with topical timolol for glaucoma developed a papular eruption on the arms and back, consistent with prurigo. All tests were within the reference ranges. There was no improvement after 1 month of topical corticosteroids. The eruption cleared completely within 1 month of timolol withdrawal. Betaxolol eye-drops were introduced and the eruption recurred within 1 week. When beta-blocker therapy was replaced by synthetic cholinergic eye-drops (drug unspecified) the eruption cleared completely without any recurrence a year later.

Other types of skin reactions that have been described in isolated reports include urticaria-like eruptions with prurigo in two patients with juvenile arthritis (23) and discoid lupus erythematosus in a woman with rheumatoid arthritis (21). Erythema multiforme in three patients and a lichenoid eruption in one were attributed to infliximab however, one patient had similar lesions after etanercept (24). 

The incidence of peripheral neuropathy, with sjmmetrical painful paresthesia of the hands and feet often accompanied by sensory loss in the legs, seems to vary with the condition that is being treated, and ranges from under 1% in patients with leprosy to over 70% in patients with prurigo nodularis (41,52). The symptoms do not correlate with either the duration of treatment or the dose. Women and elderly people seem to have an increased risk of neuropathy (53). 

Redness of the palms was observed in four of 13 patients treated with thalidomide (43). Erythroderma and peripheral eosinophilia occurred in two patients with chronic renal insufficiency who were taking thalidomide for prurigo nodularis (78). 

Many liquid and solid oral drugs contain saccharin as a sweetener. Adverse effects resulting from pharmaceuticals include pruritus, urticaria, eczema, photosensitivity, prurigo, wheezing, nausea, diarrhea, tongue blister, tachycardia, headache, and sensory neuropathy. I85 88 ... 

The pruritus of uremia is treated most effectively with UVB radiation. Prurigo, a ubiquitous disorder associated with itchy nodules of the skin, is notoriously difficult to treat. In addition to topical and intralesional steroids, prurigo may respond to the opioid antagonist naltrexone (see Chapter 21) at a dose of 50 mg/day or to the proton pump inhibitor omeprazole (see Chapter 36). 

Sulphur is much used both externally and internally in medicine. It is a mild laxative, and acts as a specific when employed as ointment in prurigo and some other cutaneous affections.

The differential diagnosis includes other types of urticaria, particularly dermographic and aquagenic urticaria. The disease should also be distinguished from abortive forms of cold urticaria such as pruritus or prurigo hiemalis (cold itch), cold erythema, cold pruritus and cold panniculitis (Henz et al. 1997).
