Infliximab (Remicade) is a chimeric monoclonal antibody directed against TNF-a. Recently, its indications have been expanded to include psoriatic arthritis and treatment of adults with chronic severe plaque psoriasis. An advantage over other systemic psoriasis treatments is that infliximab does not adversely affect blood counts, hepatic enzyme levels, or kidney function. The recommended dose is 5 mg/kg as an IV infusion at weeks 0, 2, and 6, then every 8 weeks thereafter. For psoriatic arthritis, it may be used with or without methotrexate. There are many pharmaceutical products widely used in medicine and dentistry that contain phenol, sometimes in fairly high concentrations. Bonain s local anesthetic mixture, combining menthol, cocaine and phenol, is well known, and is used, for example, as an intranasal analgesic treatment for certain facial pains. Other products containing phenol include hemorrhoid creams, chilblain solutions, ear drops or wax remover drops, psoriasis treatments, and mouth sprays with phenol in concentrations ranging from 3 mg/ml to 50 mg/g. Applying these products locally introduces only a small quantity of phenol into the organism at well-spaced intervals, and does not appear to cause intoxication. [Pg.213]

Topical treatments of first choice for mild to moderate psoriasis include keratolytics, corticosteroids, vitamin D analogues (calcipotriene, calcitriol, and tacalcitol), and tazarotene. The systemic treatment of first choice for moderate to severe psoriasis is acitretin. See Tables 96-2 and 96-3 for topical and systemic psoriasis treatment guidelines. [Pg.1772]

Systemic consequences of topical corticosteroid use include risk of suppression of the hypothalamic-pituitary-adrenal axis, hyperglycemia, and development of Cushingoid features. Avoidance of prolonged therapy with very-high-potency agents minimizes the risk of these adverse effects. Tachyphylaxis and rebound flare of psoriasis after abrupt cessation of topical corticosteroid therapy may also occur. With proper monitoring, topical corticosteroids are a safe and effective adjunctive approach to psoriasis treatment. [Pg.1774]

Topical corticosteroids are available in ointments, creams, lotions, gels, sprays, shampoos, and mousses. An ointment is considered the most clinically effective dosage form in psoriasis treatment because it consists of an oily phase that is occlusive and conveys a hydrating effect. Due to the lipophilicity of ointments, penetration of corticosteroid into dermis is enhanced, resulting in increased vasoconstriction. [Pg.1774]

The biologic agents currently used in moderate to severe psoriasis treatment are infliximab, etanercept, alefacept, and efaliznmab. [Pg.1779]

The most common adverse effects of infliximab are headaches, fever, chills, fatigue, diarrhea, pharyngitis, upper respiratory and urinary tract infections, and hypersensitivity reactions (urticaria, dyspnea, and hypotension). Infliximab has been also associated with infections and lymphoproliferative disorders. It is not associated with end-organ toxicity, and blood counts, liver enzyme levels, kidney function, and complement values can be expected to remain normal during treatment. This gives it a major advantage over other systemic psoriasis treatments. [Pg.1779]

Other reports of skin irritation after contact with coal tar (Cusano et al. 1992) were found. Patients admitted over a 4-month period to the dermatology wards of Manchester Skin Hospital for the treatment of psoriasis were patch tested with 3% coal tar (Burden et al. 1994). A positive reaction to coal tar was followed by dilution series testing down to a 0.1% concentration of coal tar. Patients were asked about previous psoriasis treatment and adverse reactions from topical preparations. Mean duration of psoriasis was 20 years and many patients had been treated as inpatients previously on numerous occasions 91% of patients recalled previous treatment with coal tar. Thirteen patients showed clinical intolerance to coal tar and two had positive patch test reaction to coal tar confirmed by serial dilutions. [Pg.136]
