Risk assessment represents a careful analysis of the premises, processes and work activities to identify what could cause harm to people to enable decisions to be made as to whether sufficient precautions have aheady been taken or whether further controls are needed. The aim of Risk Assessment it to individualize a risk priority that allows to define a hierarchy of intervention activities (design review, procedures, formation and information) needed to eliminate or reduce the risk. There are many methods for Risk Assessment (FMEA, FTA, HAZOP, What-if, MOZAR, etc.) (Hiromitsu 1996) that are all usable. [Pg.1147]

Many facilities are installing information networks to help distribute and control procedures. The primary advantage is that workers can access the most current procedures at any time for an onscreen review or can print them out for use or for training. However, electronic display has its drawbacks. Often, a procedure formatted for hard copy use does not provide a fully visible page on the monitor screen. If procedures are to be used primarily by someone on a network and are not printed out, the format should be designed specifically for the monitor display. Chemical solution deposition (CSD) procedures have been widely used for the production of both amorphous and crystalline thin films for more than 20 years.1 Both colloidal (particulate) and polymeric-based processes have been developed. Numerous advances have been demonstrated in understanding solution chemistry, film formation behavior, and for crystalline films, phase transformation mechanisms during thermal processing. Several excellent review articles regarding CSD have been published, and the reader is referred to Refs. 5-12 for additional information on the topic. Recently, modeling of phase transformation behavior for control of thin-film microstructure has also been considered, as manipulation of film orientation and microstructure for various applications has grown in interest.13-15... [Pg.33]

Operational procedures for the control of sulfide problems have played an important role for existing sewer systems over the last 40-50 years. The reason is that sulfide problems have not always been considered and predicted in the design phase, or it has been acceptable to deal with sulfide problems in the daily operation of the sewer network. However, in pressure mains, sulfide formation may typically take place. In such systems, hydrogen sulfide control may be needed, and procedures that are operated by the municipality should be implemented. Table 6.4 outlines methods that may be used for such control. Some of these methods will be further considered. The details are described in the literature. Further information of relevance in this respect is found in Melbourne and Metropolitan Board of Works (1989), ASCE and WPCF (1982), ASCE (1989), USEPA (1974, 1985), Pomeroy et al. The crucial point of the procedure is the control of the stoichiometry of the reaction between the living A chains and the DPE derivative, otherwise a mixture of stars is produced. A major problem is the fact that the rate constants for the reaction of the first and second polymeric chain with the DPE derivative are different. This results in bimodal distributions because of the formation of both the monoanion and dianion. In order to overcome this problem polar compounds have to be added, but it is well known that they affect dramatically the microstructure of the polydienes that are formed in the last step. However the addition of lithium sec-butoxide to the living coupled DPE derivative, prior to the addition of the diene monomer, was found to produce monomodal well defined stars with high 1,4 content. Finally another weak point of the method is that, as in the case of the DVB route, the B arms cannot be isolated from the reaction mixture and characterized separately. It is therefore difficult to obtain unambiguous information about the formation of the desired products. [Pg.80]
