Perioral wrinkles (around the upper lip and chin) only respond to very deep peels (Figure 5.10). Many different agents have been tried e.g. pyruvic acid, that has a high potential for scarring, and this particular agent, which is difficult to control in concentrated solutions, is very rarely used. 

Organic Acids. The ability of 33 strains of yeast to grow and produce pyruvic acid using dried citrus peel as a substrate was evaluated (22). Debaryomyces coudertii IFO 1381 and Candida utilis IF0 0396 produced yields of pyruvic acid near 82 mg/100 mL after 24 hours of fermentation at 30°C. Fermentation conditions were optimized with respect to nitrogen source (0.5% ammonium sulfate, dibasic phosphate concentration (0.1%), yeast extract concentration (0.01%), calcium carbonate concentration (1.0%), and magnesium sulfate concentration (0.01%). Pyruvic acid is a carboxylic acid having a keto group at the a position of the aliphatic carbon atom (a-keto acid). Because of its low pKa and its small dimension, it penetrates rapidly and deeply throngh the skin and is considered a potent chemical peel agent. 

A small fan should be used during the application of pyruvic acid to avoid inhaling the vapors. Moisturizing creams and sun screens must be applied after the peel.

In case of hyperpigmented spots due to inadequate sun avoidance (Fig. 23.4), prescribe bleaching agents, topical tretinoin, and topical steroids. A soft peeling with 25% salicylic acid or 40% pyruvic acid can reduce the pigmentation. 

Skin priming consists of topical application of compounds, such as retinoic acid 0.05%, glycolic acid 10%, pyruvic acid 7%, and hydroquinone 2-4%, used alone or in combination, for at least 2 weeks before the peel procedure. These agents used in the pre-peel period cause a superficial exfoliation due to keratino-cyte discohesion, and allow a more uniform, rapid, and deeper penetration [11]. 

Actinic keratoses can be selectively treated with either TCA (30%) or pyruvic acid (50-60%). In melasma, pyruvic acid can be utilized in combination with 40% salicylic acid alone or with 30% salicylic acid -I- TCA 10% gel. Pyruvic acid peels can also be used in post-inflammatory hyperpigmentation and solar lentigines.
