Occupational accidents: reporting

NOTE USE OCCUPATIONAL ACCIDENT REPORT INSTEAD IF PERSONAL INJURY WAS INVOLVED. [Pg.310]

These type of accidents were also reported in Sweden (28). Of 704 consecutive patients and 580 age-matched controls, male heavy snorers were twice as likely to have occupational accidents as compared to age-matched controls, whereas female heavy snorers had a threefold increase compared to controls. [Pg.216]

Occupational exposure to CDDs occurs primarily through inhalation and dermal contact of fire fighters and cleanup workers involved with transformers containing PCBs and polychlorobenzenes in workers involved in incineration operations in workers in metal reclamation facilities, and in workers producing and handling pesticides, hexachlorophene, trichlorophenol, or other chlorinated compounds (e g., pentachlorophenol) that may contain small impurities of 2,3,7,8-TCDD or other CDDs (Papke et al. 1992). In addition, these authors reported that the CDD/CDF homologue profiles in whole blood of workers engaged in a variety of different chemical processes or in occupational accidents exhibited distinct CDD/CDF patterns (Papke et al. 1992). [Pg.509]

The subcommittee evaluated human data from experimental, occupational, and epidemiologic studies data from accident reports and experimental-animal data (single and repeated exposures). The evaluations focused primarily on high-concentration inhalation exposure studies. The subcommittee s recommended SEALs are based solely on scientific data relevant to health effects. [Pg.23]

This chapter delves into the causation of the loss of a US. Army Black Hawk helicopter and all its occupants from friendly fire by a US. Air Force F-15 over northern Iraq in 1994. This example was chosen because the controversy and multiple viewpoints and books about the shootdown provide the information necessary to create most of the STAMP analysis. Accident reports often leave out important causal information (as did the official accident report in this case). Because of the nature of the accident, most of the focus is on operations. Appendix B presents an example of an accident where engineering development plays an important role. Social issues involving public health are the focus of the accident analysis in appendix C. [Pg.103]

The Petroleum Safety Authority (PSA) has been engaged in measuring the safety performance of the petroleum industry on the Norwegian OCS and the effects of PSA activities (see Chapter 10 by Bang and Thuestad in this volume). The Norwegian petroleum industry is required by law to report incidents that could lead to severe accidents to PSA. This is required for both major accidents and occupational accidents. The PSA and the industry have a tradition of close cooperation and sharing of experiences. [Pg.226]

In Estonia, the official statistics on occupational accidents are based on the information received in the Work Accidents Reports which are obliged to send to Sub-Bureaus of NLI by employers. The doctor is also obligated to notify NLI about the employee s severe injury or dangerous health condition as a result of the... [Pg.54]

In 2005 and 2006, the risk assessment report was carefully scrutinized and the reasons of occupational accidents discussed in-depth together with management of the company. It was concluded, that the accidents occurred in 2000-2003 were often caused by the lack of safety measures or ignorance of measures by workers while in 2004-2005 the accidents occurred due to the construction principles of certain older machines or worker s inattentiveness of the worker. It is clear, that workers operating with some specific older machine types are easier subjects to work accident and therefore, workers need to be specifically cautious and follow all the safety rules accurately until the company has been able to allocate financial funds to renew the machinery. [Pg.58]

Occupational fatahties and injuries caused by electricity pose a serious pubhc problem and in occupational accidents of the Netherlands. Contact with electricity constitutes 1.5% of the reported 12500 accidents, which have occurred in the Netherlands between 1998 and 2004, while the number of deaths, permanent and recoverable injuries is on the average 30 per year. [Pg.711]

Upon notification of an occupational injury or illness, the supervisor should complete a formal incident/accideut report aud, if possible, seud it with the injured employee to the medical professioual involved. The incident/accident report form must be completed and forwarded to the company medical department even if the employee receives medical treatment at the hospital and/or from a private physician. [Pg.78]

Specific questions about the number of occupational safety accidents there had been the year before revealed widespread ignorance. Ability to reply was structured largely according to position in the hierarchy (Chart 9.1). These differences very much reflected the contingencies of work, most particularly receipt of company reports (Ch. 7). Consideration of accident reports and statistics was part of managers workloads and they were the most confident that they saw accident reports and statistics whereas the workforce, in the context of these questions, did not refer to any accident data. The majority of safety representatives believed that they had access to the relevant accident information but not all supervisors received the accident data. Access to the data did not necessarily mean that it was read or retained. Safety representatives remarked that it could be very difficult to get through the data they received in addition to their other responsibilities. Managers claimed to read the information but... [Pg.200]

ABSTRACT Analysis of accident reports has been a useful tool in occupational safety research. Important variables related to main circumstances of accidents are being gathered in Europe according to European Statistics on Accidents at Work framework (ESAW). This paper present a method for the identification of possible public safety programs based on the application of Multiple Correspondence Analyses (MCA) and the introduction of the concepts of task and accident mechanism. The method is presented using the accidents notified in the manufacturing sector of Andalusia. This method can help policy makers in the identification of areas of public intervention. [Pg.79]

Analysis of accident reports have been a useful tool in occupational safety research (Khanzode et al., 2012). Governments have also understood the importance of the quality of accident reports data. At the end of the last century, a serious improvement took place in Europe with the development of the project denominated European Statistics on Accidents at Work (ESAW hereinafter), launched in 1990. The main purpose of ESAW is to harmonize data on accidents at work and also to incorporate meaningful variables of the accident scenarios. [Pg.79]

In Spain, accident reports are electronically collected in Official Workplace Incident Notification Forms . All accidents that result in an absence from work of one or more days must be notified. Medical criteria are applied by the physicians of the Mutual Insurance System for Occupational Injuries and Illnesses to classify the accident as slight or severe, depending on the injuries and the expected period of recovery. [Pg.80]

Farming is an activity that has multiple risk factors, thus many researchers have reported that the rate of occupational accidents occurring in farming sector is one of the highest of those obtained for the different sectors of activity (Kumar and Dewangan, 2009 HSE, 2013 Chae et al., 2014 and Svendsen et al., 2014). [Pg.123]

In Brazil there is a lack regarding specific statistical data on accidents in confined environments, despite the Ministry of Welfare and Social Assistance (MPAS) provide the Statistical Yearbook of Social Security with some data on occupational accidents. But it is suspected that such data do not represent all because just contain only accidents recorded by Communication Workers Compensation (CAT), and one of the accidents are not reported. [Pg.344]

The safety officer should analyze each accident report and classify it using a systematic method. Occupational injuries should be divided into type, source, and cause. Type refers to the kind of injury involved and usually falls into one of the following classes ... [Pg.14]
