SNVP is a lipophilic N-substituted valeryl analog of SNAP. It caused prolonged nitric oxide mediated vasodilatation in rats [70, 71]. In vivo, SNVP prevented the hyper-aggregability of circulating platelets caused by angioplasty of rabbit carotid arteries. Here, only minor effects on blood pressure were observed, whereas platelet adhesion was strongly inhibited [71].

Sildenafil is a selective inhibitor of cGMP-specific PD-5 and therefore inhibits the degradation of cGMP PD-5, the predominant type in the corpus cavernosum, also is present in other tissues (e.g., lungs, platelets, and eye). TTie selective inhibition of this enzyme facilitates the release of nitric oxide and smooth muscle relaxation of the corpus cavernosa. Sildenafil enhances erection by augmenting nitric oxide-mediated relaxation pathways. It has been suggested that sildenafil s mechanism of action is due to cross-talk between cGMP- and cAMP-dependent transduction pathways within the cavernous muscles. 

The oxidation hypothesis of atherosclerosis states that the oxidative modification of LDL (or other lipoproteins) is important and possibly obligatory in the pathogenesis of the atherosclerotic lesion thus, it has been suggested that inhibiting the oxidation of LDL will decrease or prevent atherosclerosis and clinical sequelae. LDL oxidation also has important implications for vascular health function. High concentrations of LDL may inhibit arterial function in terms of the release of nitric oxide from the endothelium and many of these effects are mediated by lipid oxidation products. How does a nonspecific effector molecule (nitric oxide) mediate the selective destruction of the /3 cell We propose that the specificity of cytokine interactions with the /8 cell, and the intrinsic sensitivity of the /3 cell to oxidative damage relative to other endocrine cells may impart this selective destruction. Nitric oxide has multiple functions depending on the amount of nitric oxide generated, the enzymatic source of nitric oxide (inducible versus constitutive), and the site at which nitric oxide is generated. If nitric oxide mediates or participates in IDDM, then inhibition of nitric oxide generation by the inducible isoform of nitric oxide synthase should delay or prevent the onset of IDDM. Many inhibitors have been developed to block nitric oxide synthase activity.
