Melanoma, malignant

The active immunotherapeutic approach is specific and based on the premise that tumor antigens are immunogenic and the host is sufficientiy immunocompetent to mount an effective immune response to an autologous tumor. Theoretically, a weak or suppressed host immune system that had allowed the formation of a tumor may be overridden by active immunization or immunostimulation. In practice, vaccines composed of so-called autologous tumor extracts have been used to treat patients with malignant melanoma (73), and purified melanoma tumor-associated antigens have been used to ehcit antibody responses in melanoma patients (74).

Light and Toxic Reactions In many individuals, exposure to ultraviolet radiation from the sun causes skin reactions such as erythema, thickening of the epidermis, and darkening of existing pigment. Exposure to ultraviolet light also increases the risk of different forms of skin cancers, especially malignant melanoma. ... 

ET-1 also stimulates anti-apoptotic signal cascades in fibroblasts, vascular smooth muscles and endothelial cells (via phosphatidylinositol-3-kinase and Akt/pro-tein kinase B). In prostate and ovarian cancer, upregulation of endothelin synthesis and ETA receptors has been associated with a progression of the disease. The inhibiton of ETA receptors results in a reduced tumour growth. In malignant melanoma, ETB receptors are associated with tumour progression. Endothelins can also stimulate apoptosis in stretch-activated vessels via the ETB receptor, which contrasts the above-mentioned effects. The molecular basis for these differential anti- and pro-apoptotic reactions mediated by endothelins remains elusive. 

Boro nophenyl) alanine (BPA) is a practical boron compound which is clinically used for the treatment of not only malignant melanoma but also of brain tumors, in neutron capture therapy (Scheme 1-40) [105, 152, 153]. Although (pinacolato)di-boron (82) is an excellent reagent to afford the corresponding boronate in 88% yield, it strongly resists the hydrolysis to arylboronic acids. Alternatively, the 1,3-diphenyl-propanediol ester (85) is more convenient to deprotect the diol moiety by catalytic hydrogenolysis [105]. 

Much attention has recently been focused on organoboronic acids and their esters because of their practical usefulness for synthetic organic reactions including asymmetric synthesis, combinatorial synthesis, and polymer synthesis [1, 3, 7-9], molecular recognition such as host-guest compounds [10], and neutron capture therapy in treatment of malignant melanoma and brain tumor ]11]. New synthetic procedures reviewed in this article wiU serve to find further appHcations of organoboron compounds. 

Staging of malignant melanoma is important to determine prognosis, categorize patients with regard to metastatic potential and survival probability, and aid in clinical decision making.
