Strong leadership is a cornerstone ofBest-in-Class safety management. Where leaders personally commit to safety and plainly communicate its importance to all members of the organization safety thrives. This is an imperative for leaders at all organizational levels, including senior corporate executives and operating site managers. 

Two members selected from the team act in supportive roles during the PHA. These roles are the leader and the scribe. A team leader is always needed for a PHA and should double as the scribe only for extremely simple hazard evaluations. For more complex analyses or the HAZOP study method, a separate person should always be assigned to scribe.

Although having the appropriate certificates of completion would satisfy regulatory requirements, you should also consider time of service and experience. A fresh college graduate with training certificates and minimal field experience may be less desirable to perform supervised field experience than the safety professional with years of substantial field experience.

The requirements of ISO 9001 do not apply solely to one department. On the part of the contractor team, two root causes were identified, root cause 1 being insufficient training of the contractor team leader. He was uncertain of permit systems, specifically whether they should report in at the end of shift, and, if so, who should do it. When individuals from several disciplines try to address a common problem, how do they communicate with each other and what process do they select for their paradigm If each individual believes that their paradigm is the only one to use and that every one must use that nomenclature and process, major confrontations can arise. If there is a dominate personality that forces everyone on the team to learn the leaders paradigm, language, and process, then a learning period of several years may be required before the team can be effective. An alternative is to create a compromise language acceptable to the team, but this can create a communication barrier between the team and the traditional disciplines. 

The session leader will need to be aware that some of the participants may be in personal circumstances that are closely similar to those in the scenarios, and will need to be sensitive to this. It may be advisable to remove a particular scenario if this is felt to be too close to home for any member of the group. 

Teachers and group leaders should be sensitive to the possibility that some participants may be adversely affected by some incidents within the story due to personal experience. 

Any situation that participants might find themselves in could present potential risks. Warning signs and labels are a common feature of everyday life but their very commonness lessens their impact. Some situations, however, come with no such warnings attached and the risks may be less obvious, but still need serious consideration. Teachers and group leaders should be sensitive to the possibility that some participants may have had personal experience of one or more of the risky situations included.
