Poisoning, lead

Care must be used in handling lead as it is a cumulative poison. Environmental concerns with lead poisoning has resulted in a national program to eliminate the lead in gasoline.

Because of the risk of lead poisoning, the exposure of children to lead-based paint is a significant public health concern. The first step in the quantitative analysis of lead in dried paint chips is to dissolve the sample. Corl evaluated several dissolution techniques. " In this study, samples of paint were collected and pulverized with a Pyrex mortar and pestle. Replicate portions of the powdered paint were then taken for analysis. Results for an unknown paint sample and for a standard reference material, in which dissolution was accomplished by a 4-6-h digestion with HNO3 on a hot plate, are shown in the following table. 

Workers who produce or use lead should be aware of possible ha2ards. Symptoms of chronic lead poisoning include fatigue, headache, constipation, uneasy stomach, irritabiHty, poor appetite, metallic taste, weight loss, and loss of sleep. Most of these same symptoms also occur in many common illnesses, such as the flu, thus a physician must rely on tests, such as blood lead analysis, to determine chronic lead poisoning. 

The alimentary symptoms may be overshadowed by neuromuscular dysfunction, accompanied by signs of motor weakness that may progress to paralysis of the exterior muscles or the wrist (wrist drop), and less often, of the ankles (foot drop). Encephalopathy, the most serious result of lead poisoning, frequendy occurs in children as a result of pica, ie, ingestion of inorganic lead compounds in paint chips this rarely occurs in adults. Nephropathy has also been associated with chronic lead poisoning (147). The toxic effects of lead may be most pronounced on the developing fetus. Consequendy, women must be particulady cautious of lead exposure (148). The U.S. Center for Disease Control recommends a blood level of less than 10 p.m per 100 mL for children.

Medical Uses. A significant usage of chelation is in the reduction of metal ion concentrations to such a level that the properties may be considered to be negligible, as in the treatment of lead poisoning. However, the nuclear properties of metals may retain then full effect under these conditions, eg, in nuclear magnetic resonance or radiation imaging and in localizing radioactivity. 

Hydroxymethyl-6-methyluracil (1043) was prepared many years ago from 6-methyl-uracil and formaldehyde, or in other ways. Since 1956 it has received much attention in the USSR under the (transliterated) name pentoxyl or pentoxil. It is used in several anaemic and disease conditions. For example, a mixture of folic acid and pentoxyl quickly reduces the anaemia resulting from lead poisoning pentoxyl stimulates the supply of serum protein after massive blood loss it stimulates wound healing it stimulates the immune response in typhus infection and it potentiates the action of sulfonamides in pneumococcus infections (70MI21300). 

Occupational disease is not a new phenomenon. Ample historical evidence exists recounting the effects of lead poisoning, chronic respiratory problems associated with mining, and hazards of manufacturing (including traumatic injury)-... 

Soon after production began, TEL was held responsible for a high incidence of illness and deaths among production workers at both the DuPont and Jersey Standard plants. The substance penetrated the skin to cause lead poisoning. Starting in late 1924, there were forty-five cases of lead poisoning and four fatalities at Jersey Standard s Bayway production plant. Additional deaths occurred at the DuPont Plant and at the Dayton Laboratory. This forced the suspension of the sale of TEL ill 1925 and the first half of 1926. 

Th ese incidents compelled the U.S. Surgeon General to investigate the health effects of TEL. The industry itself moved rapidly to deal with the crisis by instituting a series of safety measures. Lead pipes The corrosion resistance of lead is generally excellent, but it is attacked by certain waters. This is usually of little significance so far as deterioration of the pipe is concerned, but is important because of danger to health, since lead is a cumulative poison even very small doses taken over long periods can produce lead poisoning. It is for this reason that its use for carrying potable water has been discontinued. 

Lead is a toxic metal and rigid precautions against lead poisoning are essential. Sprayed lead is, like other sprayed metals, porous, and the sprayed layers will not as a rule withstand attack by strong acids. On the other hand, lead from 0-13 to 0-25 mm (0-005 to 0-01 in) in thickness has proved extremely useful in atmospheres containing sulphuric acid. In this case the pores in the lead become blocked with lead sulphate, with the result that complete protection is assured. In the event of mechanical breakdown, lead does not exhibit any sacrificial action and therefore corrosion may lift the lead layer.
