Many events involve multiple persons, often from different companies. An organization chart can be very useful and can help demonstrate the decision making process, the manner in which instructions were issued and how information was conveyed prior to the incident. [Pg.509]

ATCC issues Certificates of Analysis with cultures. Although ATCC Certificate of Analysis do not meet the specific requirements of ISO Guide 31, they do show that the biological RM culture has been authenticated and/or specific characteristics have been verified. Each ATCC Certificate of Analysis is lot-specific and includes expiration dates, the specific seeds used for propagation, and selected biochemical and morphological characteristics that are indicative of the culture. In response to post-Macondo criticism, API has created a new Center for Offshore Safety dedicated to continuous improvement in the safety of deepwater operations. But BSEE has yet to address its dependence on API and other industrial organizations for safety expertise and determine how to become the senior partner in public-private collaborations on safety issues. The occupational instruction is carried out using a manual originally created for the city of Kemi and later modified to fit the port environment. Employees of the Port of Kemi Ltd. are interviewed concerning their job satisfaction and progress once a year. No systematic reward practices exist but casual rewards such as a free lunch might be in use. A customer satisfaction study is organized once every 2 years and has always provided initiatives and ideas for improvement. The focus used to be on marketing aspects but now it is more customized and includes the HSEQ issues if relevant to the customer. Feedback is also collected. [Pg.81]
