In correlation studies, the unit of investigation is usually whole populations (eg, particular geographical areas) and cancer frequency is related to a summary measure of the exposure of the population to the agent. Because individual exposure is not documented, a causal relationship is less easy to infer from such studies than from cohort and case-control studies. Case reports generally arise from a suspicion, based on clinical experience, that the concurrence of 2 events — that is, a particular exposure and occurrence of a cancer — has happened rather more frequently than would be expected by chance. The uncertainties surrounding interpretation of case reports and correlation studies make them inadequate, except in rare cases, to form the sole basis for inferring a causal relationship. 

There are many similar studies from the US, UK, Canada, France, Sweden, Finland, etc, covering millions of people which fail to show a correlation of cancer incidence with small excesses in low-LET radiation. For example, the county in Sweden (Vastergdtland) with the highest average background radiation has the lowest total cancer frequency. 

Many studies have tried to relate lung cancer frequency with the Rn-concentrations in ordinary air ( 200 Bq/m ). Except for the study of uranium miners ( 18.11 and 18.13.5), all these investigations, though carried out with utmost care, have failed to statistically prove such a relation. However, such a relation is still assumed to exist. 

As the largest study ever undertaken to study the relationships between dietary habits and disease states including cancer, The European Prospective Investigation into Cancer and Nutrition found a reverse correlation between fluid intake and bladder cancer frequency [30]. As expected, higher caloric intake was correlated with higher incidence of bladder cancer in men older than 60 [31]. Similar results were obtained with fruit and vegetable, green tea polyphenol, and vitamin supplement consumption, despite the fact that the results were sometimes unexpected for different cancer cases [32-34].

Health problems are documented for gold miners who worked mainly underground with litUe exposure to elemental mercury in Australia, North America, South America, Europe, and Africa. Major problems examined included life expectancy, cancer frequency, and pleural diseases. Health problems of miners who worked mainly on the surface and with extensive exposure to elemental mercury owing to its use in amalgamating and extracting gold are reported extensively in Chapter 19.

Mammalian Cells Unlike microbial cells, mammalian cells do not continue to reproduce forever. Cancerous cells have lost this natural timing that leads to death after a few dozen generations and continue to multiply indefinitely. Hybridoma cells from the fusion of two mammalian lymphoid cells, one cancerous and the other normal, are important for mammalian cell culture. They produce monoclonal antibodies for research, for affinity methods for biological separations, and for analyses used in the diagnosis and treatment of some diseases. However, the frequency of fusion is low. If the unfused cells are not killed, the myelomas 1 overgrow the hybrid cells. The myelomas can be isolated when there is a defect in their production of enzymes involved in nucleotide synthesis. Mammahan cells can produce the necessary enzymes and thus so can the fused cells. When the cells are placed in a medium in which the enzymes are necessaiy for survival, the myelomas will not survive. The unfused normal cells will die because of their limited life span. Thus, after a period of time, the hybridomas will be the only cells left ahve. 

The public consequences were significantly lower public than expected for a typical commercial LWR primarily because of the large distance to the public (>6 miles) and dispersal afforded by the height of the stack. The mean frequencies per year for offsite early fatalities are Internal Events 9.7E-13, Fire 3.8E-9, Seismic 2.2E-8, and Total 3.1E-9. For offsite latent cancer fatalities the results are Internal Events - 4.1E-3, Fire - 4.2E-3, Seismic - 6.0E-2, Total - 6.8E-2. For onsite early fatalities the results are Internal Events l.OE-6, Fire 8.3E-5, Seismic 8.6E-4, and Total 9 4E-4. For onsite latent cancer fatalities the results are Internal Events 2.70-4, Fire 2 3E-4, Seismic 3.0E-3, and Total 3.5E-3. 

At typical indoor concentrations, COj is not thought to be a direct cause of adverse health effects however, COj is an easily-measured surrogate for other occupant-generated pollutants. Eye, nose, and throat irritation headaches lung cancer may contribute to heart disease buildup of fluid in the middle ear increased severity and frequency of asthma episodes decreased lung function. ETS is also a source of odor and irritation complaints.

In patients with ataxia-telangiectasia, an autosomal recessive disease in humans resulting in the development of cerebellar ataxia and lymphoreticular neoplasms, there appears to exist an increased sensitivity to damage by x-ray. Patients with Fanconi s anemia, an autosomal recessive anemia characterized also by an increased frequency of cancer and by chromosomal instability, probably have defective repair of cross-linking damage. 

All three of these chnical syndromes are associated with an increased frequency of cancer. It is hkely that other human diseases resulting from disordered DNA repair capabihties will be found in the fiimre.

An inverse correlation between thyroid cancer risk and phytoestrogens was recently proposed as a result of a multi-ethnic population-based case control study conducted in the San Francisco Bay Area (Hom-Ross et al., 2002). In this study, dietary habits and phytoestrogen consumption were assessed by a food-frequency questionnaire and by a nutrient database. The outcome of the study was that soy-based foods and alfalfa sprouts were associated with a reduction of thyroid cancer risk, whereas a Western diet did not influence cancer risk. No difference was observed between American and Asian women or between pre- and postmenopausal women. Furthermore, among the few compounds examined, the isoflavones genistein and daidzein and the lignan secoisolariciresinol were the phytoestrogens most frequently associated with risk reduction (Horn-Ross et al., 2002).

This series of trials, and many more, has led to the dramatic change in how HRT is currently prescribed and greater understanding of the associated risks. HRT, once thought of as a cure-all for menopausal symptoms, is now a therapy that should be used only to reduce the frequency and severity of vasomotor symptoms associated with menopause in women without risk factors for CHD or breast cancer. The changes that have occurred over the years in the use of HRT further support the importance of evidence-based practice and judicious medication use. 

Benign prostatic hyperplasia (BPH) is one of the most common problems of elderly men, affecting more than 40% of men over age 70. BPH results in the urinary symptoms of hesitancy and frequency. Since prostate cancer affects a similar age group and often has similar presenting symptoms, the presence of BPH often complicates the diagnosis of prostate cancer, although it does not appear to increase the risk of developing prostate cancer.2,5...
