Bismuth salicylate

Bismuth salicylate has a number of properties contributing to its antidiarrheal effects. This drug may stimulate water and electrolyte absorption from the lower GI tract, thus decreasing fecal fluid loss. In addition, the bismuth component of this compound may have antibacterial effects, and the salicylate component may inhibit the production of prostaglandins that irritate the intestinal lining. The combination of these properties makes this drug fairly effective in 

Bismuth salicylate is the active ingredient in Pepto-Bismol, a fairly inexpensive and readily available over-the-counter commercial product. 

This drug is relatively free from serious side effects. Problems with salicylate intoxication may occur during overdose or in people who are sensitive to aspirin and other salicylates.

Metals may also be linked through an oxygen or nitrogen atom to form a stable metal complex without a carbon-metal bond. These include metal complexes of ethylenediamine tetraacetate (EDTA), diethylenetriamine pentaacetate (DTPA), or ethylenediamine tetramethylphosphonate (EDTMP). Metalloid compounds include antimonyl gluconate and bismuth salicylate.

Bismuth in pharmaceuticals such as bismuth tribromophenoxide, bismuth salicylate and bismuth subgallate has been determined complexiometrically. The sample is shaken with dilute nitric acid for 5 min and then diethyl ether added with gentle swirling followed by water, and the solution is heated to 60 °C. The solution is then cooled to 50 °C, ignoring the precipitate, and a 1% solution of methyl thymol blue in potassium nitrate is added and the solution titrated immediately with 0.1 M EDTA-disodium salt to a pale red-violet colour. Then 10% aqueous ammonia is added and this solution titrated further until it becomes yellow. 

Bismuth salts are still in use. Tripotassium dicitrato-bismuthate and bicitropeptide (a bismuth-peptide complex) are used in the eradication of Helicobacter pylori in combination with antibiotics (SEDA-21,233) (1 ), and ranitidine bismuth citrate is used to treat peptic ulcer (5). Bismuth salicylates are used in other intestinal diseases, such as microscopic colitis (6,7) and collagenous colitis (8). Bismuth subnitrate plus iodoform is used to pack surgical cavities. Bismuth oxide and bismuth subgallate are found in some topical formulations that are used for treating hemorrhoids. Bismuth is also used topically as a bacteriostatic.

Bismuth compounds are added more and more to special polymers for bone implants and dental prothetic devices in order to make them detectable to X-rays. Basic bismuth salicylate is used to impart a pearly surface to polystyrene and phenol-formaldehyde resins.

Ranitidine possibly causes an increase in the absorption of bismuth from tripotassium dicitratobismuthate, but not bismuth salicylate or bismuth subnitrate. Any increase is considered unlikely to be clinically relevant with recommended short courses for H. pylori eradication. Other H2-receptor antagonists would be expected to interact similarly. 

The AUC of a single 240-mg dose of tripotassium dicitratobismuthate (TDB, De Noltabs), was increased fourfold in 12 healthy subjects given two 300-mg doses of ranitidine (one the night before and one 2 hours before the bismuth compound). The maximum serum levels were approximately doubled. The same regimen of ranitidine had no significant effect on the absorption of bismuth from bismuth salicylate (Pepto-Bismol) or bismuth subnitrate (Roter tablets) ...
