In 1989, the amino acid dietary supplement L-tryptophan was thought to he associated with an outbreak of eosinophila-myalgia syndrome (EMS), a condition characterized by an increase in white blood cell count, severe muscle pain, and other skin and neuromuscular problems. Later studies showed that an impurity present during the manufacture of the amino acid was probably responsible for the outbreak of EMS and that the amino acid itself was probably safe for human consumption. [Pg.50]

Organisms vary widely in their ability to synthesize amino acids. Dietary amino acid requirements for organisms vary from all to none. Mammals require about half of the amino acids in their diet for growth and maintenance of normal nitrogen balance (see here). Amino acids that must be provided in the diet to meet an animal s metabolic needs are called essential amino acids (Table 20.1). The nature of the diet sets the basic pattern of metabohsm. There is a need to process the products of digestion of dietary carbohydrate, lipid, and protein. These are mainly glucose, fatty acids and glycerol, and amino acids, respectively. In ruminants (and to a lesser extent in other herbivores), dietary cellulose is fermented by symbiotic microorganisms to short-chain fatty acids (acetic, propionic, butyric), and metabohsm in these animals is adapted to use these fatty acids as major substrates. As the name implies, the odor of urine in maple syrup urine disease (brancbed-chain ketonuria) suggests maple symp or burnt sugar. The biochemical defect involves the a-keto acid decarboxylase complex (reaction 2, Figure 30-19). Plasma and urinary levels of leucine, isoleucine, valine, a-keto acids, and a-hydroxy acids (reduced a-keto acids) are elevated. The mechanism of toxicity is unknown. Early diagnosis, especially prior to 1 week of age, employs enzymatic analysis. Prompt replacement of dietary protein by an amino acid mixture that lacks leucine, isoleucine, and valine averts brain damage and early mortality. [Pg.259]
