EPRI NP-5664 is a study based on interviews of personnel at 10 utilities and 15 NRC personnel regarding the usefulness of PSA (they use the term PRA - probabilistic risk assessment). The general utility motivation for using PSA is to demonstrate an acceptably low level of risk to the NRC. Some utilities applied PSA to individual systems, functions, or issues. These smaller [ir(>grams served to train a PSA cadre and introduce PSA to other utility personnel and management. [Pg.402]

The study initially predicted a small increase in worldwide turnover according to general economic development. With the plant interview information, verification of the data, and the completion of the simple calculations, an experienced troubleshooter will develop a set of hypotheses for the root cause of the defect. After the hypotheses are established, a series of experiments need to be developed that accept or reject the hypotheses. Once a hypothesis is accepted via experimentation, then the next step is to develop a technical solution to remove the defect. Often more than one technical solution Is possible. The best technical solution will depend on the cost and time to implement the solution, machine owner acceptance, and the risk associated with the modified process. For follow-up interviews and general information gathering (factfinding type meetings), the ratio of interviewee/interviewer is less critical. A group interview can come across as more open, honest, and less covert. A team atmosphere can be created. Later in the investigation, it may be acceptable to have multiple witnesses present as details and inconsistencies are resolved. The team will have to make this judgment based on the specifics of the occurrence and the workplace atmosphere. [Pg.150]

Most people do not object to having a tape recorder record their conversation, but it is always polite to ask beforehand if a tape recorder is acceptable. Donations to charities made by companies in return for health professionals attendance at company stands at meetings or offered as rewards for completing and returning quiz cards in mailings and suchlike are not unacceptable under this clause provided that the level of donation for each individual is modest, the money is for a reputable charity and any action required of the health professional is not inappropriate. Any donation to a charity must not constitute a payment that would otherwise be unacceptable under the Code. The account I heard most often in my interviews mirrors my own experience. It is one of ambivalence toward medications, of reluctance to rely on them, of movement from one drug or dose to another, and of uncertainty about the treatment s efficacy.
