For over 20 years the tank successfully operated with a nitrogen pad to reduce moisture intrusion and a simple hinged breather vent to provide overpressure and vacuum protection. The modification (installed and completed just three days prior to the failure) provided a vent compressor and a new state-of-the-art nearly zero leakage pressure/vacuum device and some unique piping features. At high pressure experiments the reactor should be installed in a pressure cell. All check valves before it, and the filter with the flow controller after it, can be kept in the vented operating room. The best way to prevent accidents such as this is to use rupture discs, which are harder to assemble incorrectly and which can be checked for correct installation after assembly. Compressed air is often used to empty tank trucks and cars. Plastic pellets are often blown out of tank trucks. When the tank is empty, the driver vents the tank and then looks through the manhole to check that the tank is empty. One day a driver who was not regularly employed on this job started to open the manhole before releasing the pressure. When he had opened two out of five quick-release fastenings, the manhole blew open. The driver was blown off the tank top and killed. [Pg.266]

Another somewhat similar incident occurred when butadiene from a reactor flowed in the wrong direction up a line used for adding emulsifier. The check valve, which should have prevented the flow, was obstructed. The emulsifier tank was in a building and had an open vent. Butadiene came out and exploded. For conventional valves, pressure drop or variations in back pressure should not exceed 10% of set pressure. Because most process safety valves are sized for critical pressure conditions, the piping must accommodate the capacity required for valve relief and not have the pressure at the end of vent or manifold exceed the critical pressure. Designing for pressure 30% to 40% of critical w ith balanced valves, yields smaller pipes yet allows proper functioning of the valve. The discharge line size must not be smaller than the valve discharge. Closed-position indicators on the valves usually satisfy a system check . These show that the valves are nominally in the closed position but do not actually prove that there is no leakage. A proving system can be met by various systems which prove the soundness of the upstream and downstream safety shut-off valves by sequential pressurization, evacuation, mechanical over-travel with limit switches and a normally open vent valve, or other means. [Pg.281]

Blowdown from the boiler(s) should always be taken to either a blowdown sump or blowdown vessel before discharging into drains.
