Fastenings, quick-release

Compressed air is often used to empty tank trucks and cars. Plastic pellets are often blown out of tank trucks. When the tank is empty, the driver vents the tank and then looks through the manhole to check that the tank is empty. One day a driver who was not regularly employed on this job started to open the manhole before releasing the pressure. When he had opened two out of five quick-release fastenings, the manhole blew open. The driver was blown off the tank top and killed.

Should not be fastened with locks other than antipanic catches or other quick releasing devices. 

The magnets installed at the Mock-Up were fastened to the drive shafts by six -in. bolts. Calculations indicated that the magnets would be considerably above tolerance radiation after long-time operation at MTR levels. To simplify the replacement of faulty magnets a quick-release mechanism has been adopted for MTR magnets.

Computers and monitors are usually not secured to work surfaces. A sudden drop from work station height would ruin the day s output for most computers, video monitors, and their managers.
