I think there is a shared mission here. Faculty who are concerned about the readiness of students should, even if they are in other departments, make themselves available to teach the students. I think we employers must also step up and help. It would be best for us to act conceitedly, rather than individually. Instead of chemical employers going to campus merely with presentations promoting our organizations, it would be better if we could cooperatively design a lecture series covering basic training issues and share the responsibility for its presentation. Students and faculty would then understand that there are common issues that employers hold important, even if we approach them in different ways.

A similar picture emerged when Jentsch, Hitt and Bowers (2002) examined the 61 aviation training issues contained within Funk and Lyall s (1997) human-automation interaction database of accident and incident reports, interviews, research reviews, and questionnaire data. Jentsch, Hitt and Bowers (2002) used an information processing model similar to that proposed by Nagel (1988) to classify automation-related problems. The results indicated that 86.9 per cent of issues were associated with perception and decision-making, while only 13.1 per cent of issues were associated with the action stage of human performance. Similarly, in air traffic control, 91.1 per cent of problems emerging from the introduction of a new air traffic control facility were classified as perceptual or decision-related in nature, while action issues accounted for only 8.9 per cent of concerns (Jentsch, Hitt and Bowers, 2002).

People unaware of policies and applicable standards. This may be a training issue, but because of its scope, it is listed separately. People working with and around electrical systems and equipment must understand the legal and organizational policies that control the work. When this is not understood, poor practices and hazardous installations persist. In addition, technical and craft people need an up-to-date, working knowledge of applicable laws, codes and consensus standards. Those carrying out training must be competent to do so. Training must include hazards, precautions and storage arrangements, also the manufacturer s misfire drill for the particular make of tool to be used. Operators should possess a current certificate of training issued by the manufacturer or by a recognised body. 

Assuming the boundaries are set in a way which enables a wide range of representative tasks to be considered then there should be sufficient information on which to derive a clear indication of latent failures for, by definition, they pervade the whole operation - limitations in, for example, training provision are unlikely to be restricted to one single training issue.

This chapter seeks to characterize and define best practices of family involvement in expanded school mental health programs. Barriers to care and strategies for the development of a framework for integrating families into their children s care will be presented. The chapter addresses unique aspects of family involvement within a school-based mental health program, presents model examples of family involvement, and describes training issues for clinicians and for successful partnerships. Finally, we explore future directions for the field and make recommendations for next steps. 

In addition to all the other reviews performed by DOE, the DOE staff reviewed selected WSRC closure packages associated with training issues and determined that all items had been adequately addressed and no additional issues remained unresolved. 

An employer in New York was cited for two serious operator training issues. The 8,400 penalty was issued for the failure to train operators and for handling loads that exceeded the trucks capacities.
