Tincture of Benzoin

Benzoin tincture contains 10% benzoin in alcohol, while the compound formulation also contains 2% aloes, 8% storax, and 4% tolu balsam in alcohol. Occasionally, allergic contact dermatitis occurs to these preparations or derivatives in Ar-ning s tincture (besides benzoin, tumenol ammonium, anthrarobin, and ether), adhesives, water repellent barrier creasms, lozenges, and cosmetics (Spott and Shelley 1970). Cross-reactions occur to balsam of Peru, storax, eugenol, vanilla, a-pinene, benzyl alcohol, and benzyl cinnamate (Hjorth 1961). [Pg.359]

Podophyllum resin, an alcoholic extract of Podophyllum peltatum, commonly known as mandrake root or May apple, is used in the treatment of condyloma acuminatum and other verrucae. It is a mixture of podophyllotoxin, and peltatin, desoxypodophyllotoxin, dehydropodophyllotoxin, and other compounds. Podophyllotoxin and its derivatives are active cytotoxic agents with specific affinity for the microtubule protein of the mitotic spindle. Normal assembly of the spindle is prevented, and epidermal mitoses are arrested in metaphase. A 25% concentration of podophyllum resin in compound tincture of benzoin is recommended for the treatment of condyloma acuminatum. Application should be restricted to wart tissue only, to limit the total amount of medication used and to prevent severe erosive changes in adjacent tissue. In treating cases of large condylomas, it is advisable to limit application to sections of the affected area to minimize systemic absorption. The patient is instructed to wash off the preparation 2-3 hours after the initial application, since the irritant reaction is variable. Depending on the individual patient s reaction, this period can be extended to 6-8 hours on subsequent applications. Benzoin resin is a local irritant. A hundred years ago, it was considered to be an aphrodisiac when ingested. An 18-year-old woman in her 34th week of pregnancy experienced severe peripheral neuropathy followed by intrauterine death of the fetus after professional application of a 25% solution of podophyllum resin in tincture of benzoin on vulval warts. A total of 7.5 ml (equivalent to 1.88 g podophyllum) was applied. The woman was under general anesthesia at the time of application. After 3 months, the woman was walking without aid and was able to type, but very slowly and clumsily.
