The use of turnover packages improves both efficiency and safety. Efficiency is improved because the process of turning over the system to operations can start quite early on, thus saving time. Safety is improved because there is a clear definition of boundaries, with a corresponding understanding of who is responsible for what. Whenever the operations department is about to assume responsibility for a turnover package, a prestartup safety review (PS SR) can be carried out on the contents of that package. [Pg.709]

Audits or some equivalent means of operational reviews are an important segment of any quality assurance program. They provide feedback to management and are the means of ensuring a continuous improvement of the system and overall facility. In the context of audits for process safety there is a close linkage with quality assurance. Quality assurance encompasses the spectrum of not only ensuring that the requirements are met, and it also provides a system to achieve continuous improvement, in addition to compliance. [Pg.317]

A type of safety identification review that methodically analyzes the interactions between individuals and machines. It reviews the operation phase to operational phase, while considering the consequences of operator-system faults at each operating step within each phase. This analysis allows for the recognition of threats from equipment faults that may coexist with operator errors. It is considered similar to a Failure Mode and Effects Analysis (FMEA), but with increased emphasis on the steps in human procedures rather than viewing hardware exclusively. Safety in the system life cycle Describes the gated review, approval, and iterative process that define how system safety is incorporated into the system life cycle, especially during design, development, implementation, and operations. [Pg.81]

The Safety Verification Tasks assure that the hazard control is validated to be appropriate and verified to be in place. Verification of the implementation of system safety processes into company operations is very important, not only for regulatory audit and inspection survival but also to be sure that money is spent wisely. Verification is done through review and approval of the design process, input to specifications that call for verification schemes, and various tests (i.e., acquisition, operational, safety) to physically test hazard control adequacy. Of course, physical inspections (destructive and nondestructive) are part of the verification process. This is extremely important in product safety, but also very important in any plant safety. Explaining the testing of safety-critical systems and testing systems to assure they operate safely is very important. [Pg.105]
