Data should be available at every phase of the service quality loop from soliciting business through client reaction and feedback. The collection and analysis of data is a means of improving the service or conversely can detect the onset of an insidious degradation of the service before it becomes a major issue.

The way many companies identify resource requirements is to solicit resource budgets from each department covering a 1 to 5 year period. However, before the managers can prepare budgets they need to know what requirements they will have to meet. They will need access to the corporate plans, sales forecasts, new product development plans, marketing plans, production plans, etc. as well as the quality policies, objectives, and procedures. 

While the term adequacy is a little vague it should be taken as meaning that the document is fit for its purpose. If the objective is stated in the document, does it fulfill that objective If it is stated that the document applies to a certain equipment, area, or activity, does it cover that equipment, area, or activity to the depth expected for such a document One of the difficulties in soliciting comments to documents is that you will gather comments on what you have written but not on what you have omitted. One useful method is to ensure that the procedures requiring the document specify the required content so that the reviewers can check the document against an agreed standard. 

The term issue in the context of documents means that copies of the document are distributed. You will of course wish to issue draft documents for comment but obviously they cannot be reviewed and approved beforehand. The sole purpose of issuing draft documents is to solicit comments. The requirement should be that the documents are reviewed and approved prior to use. Some organizations insist that even drafts are approved for issue. Others go further and insist that copies cannot be taken from unapproved documents. This is nonsense and not what is intended by the standard. Your draft documents need to look different from the approved versions either by using letter issue notation (a common convention) or by printing on colored or watermark paper. If the approved document would carry signatures, the absence of any signature indicates that the document is not approved. 

Communications should be two-way, incorporating mechanisms (e.g., questionnaires, response cards, callback numbers, etc.) for soliciting feedback from the people receiving information. This is particularly true of communications that support initiatives like PSM that require cooperation and may require changes in working habits. 

Measure and monitor. Installation at the test sites will be closely monitored against an established timetable, as well as in terms of our established PSM criteria and goals. Throughout this process, we will solicit feedback and other commentary from the pilot sites for incorporation into ongoing process improvement.

Such feedback, from anyone who interacts with the system by providing input, using information, or receiving reports, can greatly enhance the effectiveness of the new PSM system. System users may have important suggestions about how to improve or streamline a system, or may not clearly understand its importance or the part they play in its implementation. At the same time, soliciting—and acting on—user feedback helps underscore the collaborative nature of PSM cmd the fact that its success depends on the user. 

The critical incident technique was first described by Flanagan (1954) and was used during World War II to analyze "near-miss incidents." The war time studies of "pilot errors" by Fitts and Jones (1947) are the classic studies using this technique. The technique can be applied in different ways. The most common application is to ask individuals to describe situations involving errors made by themselves or their colleagues. Another, more systematic approach is to get them to fill in reports on critical incidents on a weekly basis. One recent development of the technique has been used in the aviation world, to solicit reports from aircraft crews in an anonjmrous or confidential way, on incidents in aircraft operations. Such data collection systems will be discussed more thoroughly in Chapter 6. 

If the questionnaire is to be filled in by a large number of people you will generally solicit more honest answers by assurances that the information is confidential and having the forms returned directly to your assessment team.

The models are even more helpful to the process engineer than simple plot plans. The advantages are multiplied, as with models the process engineer can study as well as solicit the advice of other engineers in visualizing a processing condition.
