When valacyclovir is used as suppressive therapy in immunocompetent individuals with genital herpes, the risk of heterosexual transmission to susceptible partners is reduced. Instruct patients to use safer sex practices with suppressive therapy (see current Centers for Disease Control and Prevention Sexually Transmitted Disease Treatment Guidelines).

Anogenital warts require specialist diagnosis and screening for other sexually transmitted diseases. Treatment is with prescription-only medications and is not considered here.

The fluoroquinolones are used in the treatment of infections caused by susceptible microorganisms. The fluoroquinolones are effective in the treatment of infections caused by gram-positive and gram-negative microorganisms. They are primarily used in the treatment of susceptible microorganisms in lower respiratory infections, infections of the skin, urinary tract infections, and sexually transmitted diseases. Ciprofloxacin, norfloxacin, and ofloxacin are available in ophthalmic forms for infections in the eyes.

Monitoring In sexually transmitted diseases when coexistent syphilis is suspected, perform darkfield examination before starting treatment and repeat the blood serology monthly for at least 4 months.

Acyclovir is useful in the treatment of herpes. Oral herpes is caused by the herpes simplex virus 1 (HSV-1), and genital herpes is caused by the herpes simplex virus 2 (HSV-2). More than 90 percent of the world s population is infected with the oral herpes virus, though there are many infected people who do not exhibit symptoms. Genital herpes is the most prevalent nondurable sexually transmitted disease. In the United States, there are about 30 million people infected with HSV-2 and an estimated 200,000 to 500,000 new cases each year. 

Spermicide-antiviral the polymer appears to be an effective delivery system for the spermicidal/antiviral agent nonoxynol-9. By its ability to adhere to vaginal tissue while retaining nonoxynol-9 in its gel structure, it is an excellent extended effect spermicide. As an antiviral, nonoxynol-9 can only kill free HIV and is not noticeably active when the virus is within a lymphocyte. In contrast, the bioadhesive gel containing nonoxynol-9 attaches to lymphocytes and maintains sufficient contact time to allow the nonoxynol-9 surfactant to disrupt the cell wall, thus eliminating the lymphocyte and killing the vims within. This suggests that the polymer may be useful in the prophylaxis of AIDS and the treatment of other sexually transmitted diseases. 

Ofloxacin Like norfloxacin, ofloxacin [oh FLOX a sin] is primarily used in the treatment of prostatitis due to E. coH and of sexually transmitted diseases (STDs) with the exception of syphilis. It may be used as alternative therapy in patients with gonorrhea. It has some benefit in the treatment of skin and lower respiratory tract infections.
