Secondary status

The statin family of six closely related hypocholesterolemic drugs are all potent competitive inhibitors of the enzyme 3-hydroxy-3-methylglutaryl coenzyme A reductase (HMG CoA reductase), the rate-limiting enzyme in cholesterol biosynthesis. The liver is their target organ, and decreased hepatic cholesterol synthesis ultimately leads to increased removal of LDL particles from the circulation. As a consequence, all other hypocholesterolemic drugs have been relegated to secondary status. 

When I interviewed the student, I discovered that no antidote had been given him once he entered the hospital. Instead, he was queried by policemen, who insisted on knowing the names of campus marijuana and LSD users. This type of treatment, in which the wellbeing of the patient is relegated to a secondary status by law enforcement personnel, has become very common as the general public s fear or psychedelics has increased.

Harassment was usually more subtle than straightforward expulsion. This particularly took the form of stopping scooters to examine the driver s licence or the machine s roadworthiness. Over the course of our experience, we have seen numerous well-written safety programs that after a brief time simply evaporated and could not even be found in an organization. The day to day pressures and priorities had allowed safety to be degraded to a secondary status.

This is particularly tme of ethylenethiourea curatives and, perhaps, secondary amine precursors often contained in sulfur modified polychloroprene types. Material safety data sheets should be consulted for specific information on products to be handled.

TBW depletion (often referred to as dehydration ) is typically a more gradual, chronic problem compared to ECF depletion. Because TBW depletion represents a loss of hypotonic fluid (proportionally more water is lost than sodium) from all body compartments, a primary disturbance of osmolality is usually seen. The signs and symptoms of TBW depletion include CNS disturbances (mental status changes, seizures, and coma), excessive thirst, dry mucous membranes, decreased skin turgor, elevated serum sodium, increased plasma osmolality, concentrated urine, and acute weight loss. Common causes of TBW depletion include insufficient oral intake, excessive insensible losses, diabetes insipidus, excessive osmotic diuresis, and impaired renal concentrating mechanisms. Long-term care residents are frequently admitted to the acute care hospital with TBW depletion secondary to lack of adequate oral intake, often with concurrent excessive insensible losses. 

The heart is located underneath the sternum and between the lungs. In most adults, it weighs between 250 and 350 grams (0.55-0.77 lb). Its major role is to provide oxygen-rich blood to all organs and areas of the body. As a secondary function, the heart secretes atrial natriuretic factor (ANF), a hormone that regulates blood pressure and volume status. The inability of the heart to pump enough blood to maintain normal body requirements is known as heart failure. Refer to Chapter 3 for more information on heart failure. 

Assess patient risk factors for osteoporosis, with special attention to age, menopausal status, previous history of osteoporotic fracture, smoking status, low body weight, family history of osteoporotic fracture in first-degree relatives, and presence of secondary causes of osteoporosis.
