Safety performance significance

The ultimate goal of PSM is improved safety performance. In addition, over and above the mandatory compliance with OSHA standards, there are potentially significant business benefits to implementing PSM, including improved efficiency, cost savings, and competitive advantages. 

Chapter 2, the committee examines the more significant of the chemical events at JACADS and TOCDF to determine their characteristics with respect to facility performance and human performance. How these events are related to safety performance is not a simple question. In his widely referenced book (Reason, 1997), in a chapter devoted to the relationship between frequent, low-consequence events and the risk of high-consequence events, James Reason concludes that ... 

At JACADS, significant progress was made in developing a safety culture, and during the latter phases of demilitarization operations the plant was consistently achieving excellent safety performance. This does not appear to have been the case at TOCDF (NRC, 1999a). 

But using time as the measure does of course become a big problem should the year be reached and the target not be achieved. Despite making significant improvements in overall safety performance, Balfour Beatty did not achieve Zero Harm by 2012, and indeed have since revised its programme, omitting any timescale from its strapline. 

As can be seen from Fig. 5, in the second sixth simulation, the impact of human factors on safety performance whose time cut down more than six months to achieve an expected safety performance is the most obvious. The next impact is management factors. The adjustment of other three factors is no significant difference. The impact of human and management factors on safety performance become more and more strong. Because staff s increased safety awareness can build safety culture, which can t be matched by other physical factors. 

Though the analysis on the number of accidents and safety checklist for each individual correlation, the presentative safety performance and safety training showed a significant decreasing function relationship. And it explained the safety training impact on the number of accidents with safety... 

The inspection programme of the regulatory body should include provision for direct monitoring of SSCs, human factors significant to safety (performance of operating personnel, managerial attitudes), tests and other safety related activities carried out by the operator. 

From 1933 to 1999, the death rate dropped 90%. It is recognized that fatality rates are not the only measure of safety performance. Nevertheless, these numbers are significant. 

In the second edition of this book, I wrote that a renewed interest had developed in having measurement systems that effectively assess occupational safety performance, measurements that are universally applicable. Preferably, those measures would not only be historical but also predictive and serve as a base from which to prioritize future safety efforts. A significant goal was to have those measures communicate well in terms that managements understand. This renewed interest in performance measures arose out of the increased desire of some environmental, safety, and health professionals to move the profession forward by being able to establish more definitively the value of their work in relation to organizational goals. 

The enterprise analyzes and prioritizes potential functional failure modes to define failure effects and identify the need for fault detection and recovery fimctions. Functional reliability models are established to support the analysis of system effectiveness for each operational scenario. Failures, which represent significant safety, performance, or environmental hazards, are modeled to completely understand system impacts. 

Moreover, analyses of new entrants are subject to significant caveats. First, they are based on very few accidents—e.g., three fatal accidents among new entrants in the Barnett and Higgins database, five total accidents among new entrants in the data used by Oster et al. In addition, as noted by Rose (1992, pp. 84-85), the results are sensitive to the measures of safety performance employed in the study, and also may depend on the definition of entrant carriers... 

One individual who had formerly worked at the FAA argued that maintenance practices had actually improved after deregulation, but that this improvement was in part regulatory driven. He described a cycle in which the airlines develop new maintenance practices as a result of operational experience, and those improved practices are eventually encoded in regulations. Thus, an innovation that may have begun at one or a few airlines is eventually mandated for all airlines. This respondent believed that the greater sophistication of preventive maintenance practices today had unquestionably helped the safety performance of the aviation industry. He cited corrosion as an area where such improvements had a significant impact. As a result of extensive efforts to understand the causes and effects of corrosion, the skins of a number of aircraft had been replaced for preventive maintenance purposes.
