The mixture of methotrexate sodium with cytarabine, fluorouracil, hydrocortisone sodium, and prednisolone sodium phosphate produces precipitate upon storage, although it is not observed immediately. A review of the literature shows that photochemical reactions generally follow first-order kinetics. However, pseudo first-order, zero-order, and even fractional-order kinetics have been reported. High-dose chemotherapy with cyclophosphamide, vincristine, prednisolone, and intrathecal methotrexate given for post-transplant lymphoproliferative disease was suggested to have favored the occurrence of acute ciclosporin neurotoxicity (headache, fever, seizures, and visual agnosia) in a 9-year-old cardiac transplant patient (239). Ciclosporin serum concentrations were normal and a further similar episode occurred on ciclosporin readministration. 

Five of thirty-two patients treated with the alternating drug regimen CAMBO-VIP (cyclophosphamide, doxorubicin, methotrexate, bleomycin, vincristine, etoposide, ifosfamide, and prednisolone) for non-Hodgkin s lymphoma developed blisters under the thickened skin of the palms and/or soles, followed by desquamation (28). 

In a double-blind, placebo-controlled study of the safety and efficacy of methotrexate therapy combined with glucocorticoids in patients with giant cell arteritis over 24 months, adverse events were defined as a new diagnosis of any condition during treatment (39). The combination of methotrexate plus prednisolone reduced the number of relapses and improved the course of the disease. Methotrexate was withdrawn in three patients who had adverse events that were clearly drug-related. One had leukopenia, anemia, and mucositis, one developed pancytopenia, and one oral ulcers. These patients were not taking folic acid or folinic acid supplements.

Methotrexate clearance may be modestly reduced by the longterm use of prednisolone, but methotrexate does not alter prednisolone pharmacokinetics. Limited evidence su ests methotrexate may alter prednisone levels. Dexamethasone may increase the acute hepatotoxicity of high-dose methotrexate.
