Phenobarbitai sodium - Phenobarbital sodium may be administered IM or IV as an anticonvulsant for emergency use. When administered IV, it may require 15 minutes or more before reaching peak concentrations in the brain.

Bicchi and Bertolino [193] analyzed a variety of pharmaceuticals for residual solvents. Samples were equilibrated directly or dissolved in a suitable solvent with a boiling point higher than that of the residual solvent to be determined. Equilibration conditions were 90 or 100°C for 20 min. A Perkin-Elmer HS-6 headspace sampler was used. The chromatographic phase chosen was a 6 x Vs in. column packed with Carbopack coated with 0.1% SP 1000. Residual ethanol in phenobarbital sodium was determined by a direct desorption method. An internal standard, /-butanol, was used. Typically, 0.44% of ethanol was detected (compared to a detection limit of 0.02 ppm). The standard deviation of six determinations was 0.026. Pharmaceutical preparations which were analyzed by the solution method included lidocaine hydrochloride, calcium pantothenate, methyl nicotinate, sodium ascorbate, nicotinamide, and phenylbutazone. Acetone, ethanol, and isopropanol were determined with typical concentrations ranging from 14 ppm for ethanol to 0.27% for acetone. Detection limits were as low as 0.03 ppm (methanol in methyl nicotinate).

SAFETY PROFILE Poison by intravenous and intraperitoneal routes. Mildly toxic by ingestion. The vitamin is destroyed by alkalies and alkaline drugs such as phenobarbital sodium and by oxidizing and reducing agents. When heated to decomposition it emits very toxic fumes of HCl, cr, SOx, and NOx. 

Incompatible with phenobarbital sodium, diazepam solution at a pH 5, ° some binary powder mixtures, lansoprazole, and formaldehyde. Acids will dissolve magnesium carbonate, with the liberation of carbon dioxide. Slight alkalinity is imparted to water. Magnesium carbonate was also found to increase the dissolution of acetazolamide formulations at a pH of 1.12 however, dissolution was retarded at a pH of 7.4. ... 

Interactions are not always visible. The formation of visible precipitates depends to a large extent on the insolubility of the two combining species in the particular mixture and the size to which the precipitated particles grow. One might assume that in an atropine and phenobarbital mixture, a barbiturate-atropine complex may precipitate. Atropine base is soluble to the extent of 1 part in 460 parts of water. There is only 0.6 mg atropine in a 5 cm dose and it is therefore well within its solubility limit. The solubility of phenobarbital is 1 mg cm . The 15 mg of phenobarbital sodium (13.5 mg of phenobarbital) which would result if all the sodium salt were to be precipitated would be in excess of its solubility. Only 0.4 mg of phenobarbital is precipitated by 0.6 mg of atropine sulfate, however, and the phenobarbital therefore remains in solution also. There is thus no precipitation. 

All prescription-only medicines may be supplied by emergency supply at the request of a practitioner, except controlled drugs in Schedules 1, 2 and 3 of the Misuse of Drugs Regulations (apart from phenobarbital or pheno-barbital sodium for the treatment of epilepsy). Please note that the emergency supply of phenobarbital or phenobarbital sodium for the treatment of a medical condition other than epilepsy would therefore be unlawful. 

The emergency supply of Schedule 1, 2 and 3 controlled dugs is prohibited, except phenobarbital and phenobarbital sodium for the treatment of epilepsy.
