Churg-Strauss syndrome A systemic necrotizing vasculitis with prominent lung involvement including severe asthma and eosinophilia. 

E. TPE is caused by microfilariae in the lungs and hyperimmune responsiveness to bancroftian or malayan filariasis. Paroxysmal respiratory symptoms may fluctuate in severity. Eosinophilia, almost always present, is usually very high, and the absence of microfilariae in the blood does not rule out TPE. A presumptive clinical diagnosis can be made by response to therapy without a lung biopsy. Diethylcarbamazine for 14 days is an effective therapy that can be repeated if symptoms persist. The role of ivermectin in TPE has not been established. 

A 24 year old man developed a fever and a generalized maculopapular rash after taking amfebutamone 150 mg bd for 3 weeks (28). Amfebutamone was withdrawn but he went on to develop angioedema, eosinophilia, and a systemic syndrome with hepatitis, myositis, and obstructive lung disease. His symptoms resolved over several weeks with a glucocorticoid. 

Various studies have also examined animal models of pulmonary inflammation that are representative of primary eosinophil or neutrophil infiltration. Lung inflammation characterized by eosinophil influx has been used as a model of asthma and is not generally associated with lung fibrosis. After several episodes of repeated antigen challange, a subset of Ascaris -sensitive Cynomolgus monkeys developed a persistent eosinophilia and enhanced intercellular adhesion molecule-1 (ICAM-1) expression on pulmonary endothelial and epithelial cells when compared to control animals (Gundel etal., 1991, 1992).
