Stimulant laxatives Bisacodyl, 5-15 mg daily. Bisacodyl (9) is a contact laxative that may be given orally or rectaHy. It is often used for evacuation of the bowel prior to surgery or diagnostic examination. It may obviate the need for a cleansing enema. 

Slow-transit constipation can be treated with chronic administration of osmotic laxatives. Tegaserod maleate 6 mg orally twice daily is an acceptable treatment. Senna, bisacodyl, and other stimulants should be used only when the others fail to deliver the desired effect.

For most nonhospitalized persons with acute constipation, the infrequent use (less than every few weeks) of most laxative products is acceptable however, before more potent laxative or cathartics are used, relatively simple measures may be tried. For example, acute constipation may be relieved by the use of a tap-water enema or a glycerin suppository if neither is effective, the use of oral sorbitol, low doses of bisacodyl or senna, or saline laxatives (e.g., milk of magnesia) may provide relief. 

Dulco-lax containing bisacodyl (stimulant laxative) is available as 5 mg tablets. 

Dulco-lax tablets containing bisacodyl, a stimulant laxative, must be swallowed whole with water and not chewed to decrease occurrence of abdominal cramps. 

Bisacodyl is a stimulant laxative that does not take long to act and is therefore useful in acute constipation. The bulk-forming laxative ispaghula husk takes longer to act when compared with bisacodyl but is useful for long-term administration. Lactulose, which is an osmotic laxative, has a lag time of about 48 hours before onset of action. Loperamide and kaolin and morphine mixture are antidiarrhoeals used in acute diarrhoea. 

Diphenolmethane derivatives (p. 177) were developed from phenolphthalein, an accidentally discovered laxative, use of which had been noted to result in rare but severe allergic reactions. Bisac-odyl and sodium picosulfate are converted by gut bacteria into the active colon-irritant principle. Given by the enteral route, bisacodyl is subject to hydrolysis of acetyl residues, absorption, conjugation in liver to glucuronic acid (or also to sulfate, p. 38), and biliary secretion into the duodenum. Oral administration is followed after approx. 6 to 8 h by discharge of soft formed stooL When given by suppository, bisacodyl produces its effect within 1 h.

The two most important diphenylmethanes are phenolphthalein and bisacodyl. Senna and cascara are the sources of anthraquinone laxatives. However, although still available in some countries phenolphthalein is now removed from most markets because of concerns over carcinogenicity. As recent as 2006 the United States Food and Drug Administration (FDA) has categorized castor oil as generally recognized as safe and effective (GRASE) for over-the-counter use as a laxative. On the other hand, in 2002 cascara was banned by the FDA. 

Constipation bulk-forming laxatives by increasing faecal mass tend to soften stools and relieve constipation, and have value in a range of symptomatic problems associated with anal fissure, haemorrhoids, and with ileostomy and colostomy dysfunction. Faecal softeners, lactulose and macrogols (polyethylene glycol) retain fluid in the bowel. Stimulant laxatives, such as the anthraquinone, senna, and bisacodyl, increase motility and can cause colic verdoses can cause diarrhea and electrolyte depletion. Chronic treatment for constipation is seldom needed, but may be in children with a tendency to faecal impaction, specialist advice should be sought. 

Nalidixic acid (l-ethyl-7-methyl-l.8-naphtlmdine-4-one-3-carboxylic add), many tradenames (e.g.. Nalidicron ), is an antibacterial. Bisacodyl [4,4 -(2-pyridylmethylene)diphenol diacetate], tradename Dulcolax, is a laxative. 

Bulk laxatives are really a more long-term solution. Bisacodyl may be an alternative stimulant laxative, but is likely to have similar adverse effects.
