In 1998, the north American Society of Pacing and Electrophysiology (NASPE), now the Heart Rhythm Society (HRS), convened a policy conference that confirmed the Byrd clinical indication scheme. This scheme has evolved into the familiar three classes of categories (Table 6.4). The policy conference recommendations for extraction of chronically implanted pacing and defibrillator leads with respect to indications, training, and facilities were formerly published in 2000 (36).

In addition to the circuit breaker, there have been a number of other SMA appHcations for various functions in electric power generation (qv), distribution, and transmission systems. One such device is a thermal indicator that provides a signal visible from the ground of a hot junction or connector in a distribution yard. Such hot spots occur as a result of the loosening of bus bar connectors owing to cycHc temperature as the electric load varies. In addition to the use of SMA flags as a hot-spot indicators, actuators that automatically maintain the contact force in a bus bar connection have been demonstrated. Based on a BeUeviHe washer fabricated from a Cu—Al—Ni SMA trained to exhibit two-way memory, these washers, when heated by a hot joint, increase their force output and correct the condition. A 30 mm diameter washer 3 mm thick can produce a force of over 4000 N. Similar in purpose... 

If a toller has a contract to perform the same toll repeatedly, but on an infrequent basis, training should not be overlooked in preparation for restarting the repetitive toll. It is a good practice to provide refresher-training sessions to assure that the personnel have received current training. One company has picked a six-month period between campaigns as the indicator that the operations team needs refresher training. 

This condition is probably misalignment, indicating a maintenance problem. The mechanic should be trained to correct this. Follow correct alignment procedures, as well as correct bolt torque procedures. 

The major inspections should also include detailed inspections of any couplings in the train. Gear couplings should be disassembled and teeth inspected for indications of problems. The most common failures encountered with continuous lubrication-type gear couplings are ... 

Side studies are often required for major decisions, and a good example is a study of gas versus steam turbines for LNG plants (Reference 5). Indications are that higher capital investment is associated with gas turbine drivers than with steam turbines for liquefaction train service, but that the added capital investment for gas turbines shows... 

If the Sohre approach is taken in a philosophical light, it clearly indicates the need for a robust design. This is true for the basic equipment as well as the installation of foundations and piping. All of the factors were considered in the approach Sohre presented, even if in some cases, they may have been considered subjective. The key to the robust approach can be simplified as low deflection. On a rotor, the shorter the bearing span and/or the heavier the shaft cross section, the lower the deflection. This tends to increase the reliability aspect. On a foundation, heavier sections tend to have lower deflection, which contributes to the ability to maintain train alignment. Better alignment is one of the factors that contributes to higher reliability.

However, the optical train illustrated in Figure 22B allows the determination of fluorescence quenching. The interfering effect described above now becomes the major effect and determines the result obtained. For this purpose the deuterium lamp is replaced by a mercury vapor lamp, whose short-wavelength emission line (2 = 254 nm) excites the luminescence indicator in the layer. Since the radiation intensity is now much greater than was the case for the deuterium lamp, the fluorescence emitted by the indicator is also much more intense and is, thus, readily measured. 

The object of the studies is to compute the indices and then take action to reduce common cause variation by preventive maintenance, mistake-proofing, operator training, revision to procedures and instructions, etc. 

ISO 10012 requires that the integrity of measuring equipment be confirmed at appropriate intervals established on the basis of stability, purpose, and usage. With new equipment it is customary to set the frequency at 12-month intervals unless recommended otherwise by the manufacturer. Often this frequency remains despite evidence during calibration that accuracy and precision is no longer stable. Such action indicates that the calibration staff have not been properly trained or that cost rather than quality...
