JHA Review Form

Supervisors and enq)loyees must be aware that there are eleven main categories of potential injury-causing situations. One or more of the following eleven ways an injury could take place is to be added to the middle column of the JHA. It is possible for a particular step to be free of hazards. If this is discovered during the review process, sinq)ly place dashes or the word none on the form. [Pg.287]

Struck by (SB). An employee or someone else may be struck by a part, vehicle, falling object, or moving object. Struck by usually inq)lies that something is in motion and the worker(s) can be struck by the object. As an exanq)le, an enq loyee can be struck by a forklift. [Pg.287]

Struck against (SAG). A struck against injury inq lies that the worker is somehow in motion and strikes against an object. For exanq le, an employee struck his head against racking while reaching in for a part. [Pg.287]

Contacted by (CBy). A contacted by injury occurs when something in motion, such as a chemical splash, makes contact with the worker. This could occur at the battery recharge station when a worker is contacted by a sulfuric acid splash. [Pg.287]

Contact with (CW). In this case the worker has made contact with a hot surface, sharp edge, an exposed electrical wire, or a chemical. The hazard may or may not be visible. This could happen when a worker makes contact with an exposed razor blade while reaching for a pencil in the desk drawer. [Pg.287]


Figure 17-4 identifies a JHA review form. Periodically, JHAs must be reviewed for accuracy, quality, and changes in the job, and evaluated in the event of an injury. If the JHA is in need of revision, affected employees are to be a part of the review and revision process that is documented on this review form. [Pg.285]
