Immunosuppresant

Corticosteroids induce a non-specific immunosuppresion. Owing to their overwhelming incidence of adverse events, many practitioners attempt to use low-dose maintenance therapy or, in some cases, complete steroid withdrawal. Corticosteroids are also effective in reversing acute rejection. 

Vaccines Vaccinia virus is a live poxvirus that can lead to strong cross-protection to smallpox for about five years, plus partial protection for ten or more years. If your are old enough, or have been in military service in recent years, you may have been vaccinated with this vaccine administered by dermal scarification, or intra-dermal jet injection. However, certain persons should NOT receive smallpox vaccine, including persons who are pregnant, persons who underwent a clinical immunosuppresion, persons with eczema, or persons with leukemia or lymphoma. 

Incidence of fungal bone and joint disease is increasing with the increase in the prevalence of factors predisposing to fungal arthritis. These are the use of central venous catheters, broad-spectrum antibiotics, immunosuppresives and abdominal surgery.

Since its introduction into clinical use in about 1979 the immunosuppresant cyclosporin has been responsible for a revolution in human organ transplantation.3 The exact mechanism of action in suppressing T-lymphocyte-mediated autoimmune responses is still not completely clear, but cyclosporin, a cyclic lipophilic peptide from a fungus, was found to bind to specific proteins that were named cydophilins.d Human cyclophilin A is a 165-residue protein which associates, in the crystal form, as a decamer with five-fold rotational and dihedral symmetry.6 This protein is also found in almost all...

Inhibitors. Aside from its role in providing reduced folate coenzymes for cells, this enzyme has attracted a great deal of attention because it appears to be a site of action of the important anticancer drugs methotrexate (amethopterin) and aminopterin.293 364 365 These compounds inhibit dihydrofolate reductase in concentrations as low as 10 8 to 10 9 M. Methotrexate is also widely used as an immunosuppresant drug and in the treatment of parasitic infections. 

A second enzyme (of mass 100 kDa) is needed for activation of phenylalanine. It is apparently the activated phenylalanine (which at some point in the process is isomerized from l- to D-phenylalanine) that initiates polymer formation in a manner analogous to that of fatty acid elongation (Fig. 17-12). Initiation occurs when the amino group of the activated phenylalanine (on the second enzyme) attacks the acyl group of the aminoacyl thioester by which the activated proline is held. Next, the freed imino group of proline attacks the activated valine, etc., to form the pentapeptide. Then two pentapeptides are joined and cyclized to give the antibiotic. The sequence is absolutely specific, and it is remarkable that this relatively small enzyme system is able to carry out each step in the proper sequence. Many other peptide antibiotics, such as the bacitracins, tyrocidines,215 and enniatins, are synthesized in a similar way,213 216 217 as are depsipeptides and the immunosuppresant cyclosporin. A virtually identical pattern is observed for formation of polyketides,218 219 whose chemistry is considered in Chapter 21.

The natural history of Graves disease is of alternating remission and relapse. Progression to hypothyroidism can occur, especially after 1 treatment. Such patients should have long-term follow-up, and are likely to require thyroid hormone replacement therapy Severe forms of thyroid eye disease should be treated with steroids and immunosuppresants or low-dose radiotherapy. Urgent surgical decompression can be required for eyophthalmos. 

Inhibition of oxazolone-induced delayed-type hypersensitivity is used as an in vivo test for inhibition of the cellular immune response. The shaved abdominal surfaces of groups of mice are sensitized by topical application of 0.1ml of 5% oxazolone. The test drug is given intraperitoneally an hour later and then daily for five consecutive doses. After an additional 4 days, the animals are challenged by application of 0.5 ml oxazolone to one ear. Ear thickness is measured 24 h later. A decrease in ear thickness relative to untreated controls is an indication of immunosuppresion. 

Pretreatment with a polysaccharide E. purpurea extract was found to decrease morbidity and mortality in mice infected by C. albicans immunosuppresed with cyclophosphamide and cyclosporine A. They found that macrophages in the Echinacea group produced an increased amount of TNF-cx. The authors state that this led to an increased resistance toward Listeria monocytogenes, C. albicans, and the intracellular parasite Leishmania enrietti (17).
