Solutions of cefepime, like those of most -lactam antibiotics, should not be added to solutions of ampicillin at a concentration greater than 40 mg/mL, and should not be added to metronidazole, vancomycin, gentamicin, tobramycin, netilmicin sulfate, or aminophylline because of potential interaction. As mentioned earlier, pharmacokinetics is not solely the property of a drug but instead is the consequence of interactions between the drug and the physiology of the patient. Thus, statements like the half-life of gentamicin is 2 hours are not very useful, as the half-life is likely be longer or shorter in a given individual patient. 

Toxicity is most likely in tissues that interact with the drug. For example, gentamicin is polycationic and binds to anionic phospholipids in the cell membranes of renal proximal tubular cells, where it inhibits phospholipases and damages intracellular organelles. 

NaCIO is an acknowledged novel absorption enhancer for ampicillin sodium [99], glycyr-rhizin [100,101], gentamicin [102], phenoxymethyl penicillin [103], cefoxitin sodium [104,105], and acyclovir [106], Takahashi et al. [107] reported that the enhanced membrane permeability of phenolsulfonphthalein depends on the disappearance kinetics of CIO from the loop and its calcium ion sequestration capacity. The enhancing mechanisms of NaCIO are proposed to be involved in (1) Ca2+ sequestration, (2) increase in pore size and solvent drag, (3) interaction with membrane proteins and lipids, and (4) increase in the intracellular calcium level [104,105,108-111],... 

Tetracycline has been shown to bind to gastrointestinal mucus by hydrogen-bonding and via electrostatic and hydrophobic interactions. It has been suggested that cephaloridine and gentamicin bind intestinal mucin via ionic interactions. In addition, gentamicin has been shown to bind to sputum and tobramycin binds to glycoprotein- and DNA-rich fractions of sputum.
