The PSM Standard requires that the following information be contained within the PSI element-physical data, reactivity data, corrosivity data, thermal and chemical stability data, and hazardous effects of potential inadvertent mixing of different materials. The standard does not specifically define what is to be included in any of these data categories, the level of detail required, or the method of compilation.41 It does, however, stipulate that an MSDS can be used to compile the data to the extent that it contains the information required. In 1996, OSHA issued a Hazard Information Bulletin cautioning that MSDSs do not always contain information about hazards from mixing or blending chemicals (OSHA, 1996). 

Within the Market Data category, the best information was provided by the Australian Bureau of Statistics (Refs. MD5, MD6, MD7). These provided current Australian production figures and export/import details. 

All boundaries, methodologies, data categories, and assumptions are clearly stated, comprehensible, and visible. 

The Metals Crystallographic Data File (CRYSTMET) covers metals, alloys, and intermetallides (77 242 entries in 2003, ca. 2500 added annually). It was started in I960 in Los Alamos (USA), in 1974 has been transferred to the National Research Council of Canada and since 1996 is managed by Toth Information Systems (Ottawa, Canada). CRYSTMET contains the bibliographic and crystallographic data categories similar to CSD and ICSD, but formal connectivity is not applicable. 

While all GxP data should be checked for correctness, it may be possible to justify a sample check for other data categories if a business case can be made to justify the omission of checks on all records. Some regulators require a double check for GxP data entry. Such checks should immediately follow the data entry and precede any further processing of the data. Where this is not possible, checking must be conducted as soon as possible and a risk assessment performed to address the potential consequences of erroneous data input. 

Confident identification—Sufficient data categories exist to preclude all but the most closely related structures. 

Tentative identification—Data categories are consistent with a particular class of molecule only.

Level 1 data categories cover nine areas. A six-digit numeric coding system (similar to MTM-V) is used to provide a detailed description of the operation being studied, each of which is documented with a specific MTM-1 motion pattern. Level 2 data are directly traceable to Level 1 rmd to MTM-1, which covers the same activities at a combined motion level. Distance ranges are reduced to one, and elements use simplified alphanumeric and mnemonic codes.

Each of the safety performance measurement methods can be characterised as a combination of the following three facets (a) data source (b) identification procedrrre and (c) data category. Typical data sources for safety performance in healthcare are patient case records, incident reports, field data (for example, obtained by observation), and self-reported data (typically elicited by the use of a questiormaire or interview). 

Various data categories are necessary to perform LCA using the Simapro software [74] these comprise, for instance, the yield of the total process, the allocation factors, input processes from the technosphere, which represent materials and energy used in the study system, emissions to soil, water bodies, and air, and information about waste treatment facilities for the several waste streams. The classification of a substance as a Category 1 carcinogen is done on the basis of epidemiological and/or animal data. This classification is further distinguished on the basis of whether the evidence for classification is largely from human data (Category lA) or from animal data (Category IB) ... 

From these two data categories a criticality matrix as detailed in Chapter I (in the name of risk matrix) is formed. In many cases, both probability of occurrences and severity categories are numbered in the scale of 1—10 and a criticality matrix can be formed for the same purpose. As these are already covered in Chapter I they are not repeated here. Analysts need to understand that these are for reference only. Analysts use their judgment of failure mode frequency for each specific application. The analyst should tailor the analysis to focus on significant components or subassemblies where failures will result in undesirable system-level effects. Based on these judgments, analysts develop risk categories for the specific application.
