Accidental exposure to 2,3,7,8-TCDD in a 1949 explosion in a trichlorophenol plant in Nitro, West Virginia, resulted in an outbreak of severe chloracne. Moses et al. (1984) conducted a cross-sectional survey of workers in this plant in 1979. In reviewing the impact of the accident, the authors indicated that 117 workers had severe chloracne as a result of the explosion however, 111 additional workers were found to have had chloracne prior to the explosion. A cross-sectional study of 226 workers in 1979 indicated that 52% had chloracne which persisted for 26 years, and in 29 subjects it was still present after 30 years. Blood levels were not measured, but the air dust in the plant was suspected to have contained 2,4,5-T contaminated with 6 ppm 2,3,7,8-TCDD compared to 0.1 ppm in later years. Similarly, high incidences of chloracne were also found in other facilities (Jirasek et al. 1976 May 1973 Poland et al. 1971 Vos et al. 1978). Appearance of chloracne after accidental occupational exposure may be immediate or delayed since workers may not always be removed from the work environment, the duration of exposure and total exposure is difficult to assess. [Pg.60]

The basic Nordic School approach, that of assessing satisfaction with a cross-section survey or appraisal raises one important dilemma. A criticism of the approach is that, when analysts and stakeholders obtain a set of scores, they may have difficulty in understanding their meaning and lack a context of interpretation. This issue is at the core of the charge made by Parasuraman and colleagues that the expectations approach is superior because it offers valuable contextual information (Parasuraman et al, 1985, 1994b). The major advantages of cross-sectional surveys are that they are relatively inexpensive and easy to design and can evaluate multiple exposures and outcomes. The major disadvantages are that they provide limited information for establishing causality because they are looking at prevalence cases and do not establish a time sequence of events. In a cross-sectional survey of Yemeni adults the self-reported frequency of khat use and psychological symptoms was assessed using face-to-face interviews with members from a random sample of urban and rural households (10). Of 800 adults surveyed, 82% of men and 43% of women had used khat at least once. There was no association between khat and negative adverse psychological symptoms, and khat users had less phobic anxiety (56%) than non-users (38%). The authors were surprised by these results and offered several explanations that the form of khat used in Yemen is less potent than in other locations that prior reports of khat-related psychosis occurred in users in unfamiliar environments that the sampling procedure may have under-represented heavier khat users and that their measurement tool was not sensitive enough to detect psychological symptoms. [Pg.560]
