Contractors Evaluations - When a contractor is evaluated either for screening purposes or a specific contract award, the information developed must be documented for future reference. 

Prepare the technical bid package, develop bidders qualifications, and contractor evaluation criteria. Lead the technical evaluation effort. 

At the end of each project, facility and contractor representatives should thoroughly evaluate the contractor s HSE performance. These postwoik evaluations should be used for future prequalifying and selection of contractors. Evaluations should be dome at or near the completion of work or upon the expiration of contracts. On many projects, incinerator contractor evaluation and selection under step 3 involve no more than a solicitation of prices from a random listing of vendors with the award of a contract to that firm proposing a system for the least cost. There are two basic problems with this approach. First, the selected incinerator contractors are assumed to have equivalent capabilities and qualifications. Second, least cosf acceptance assumes that the equipment offered by each of the contractors is equivalent or identical. A comparative value assessment of proposals usually results in the procurement of a superior quality system for a negligible price difference. It is not uncommon to see cost proposals low by no more than 10%, but the equipment offered of only half the quality of the competition. 

Following the development of the acceptance criteria, the DOE staff and their support contractors evaluated the results of the DOE and operating contractor (WSRC) activities to meet these criteria. The results of those evaluations are documented in this report. Deviations or failures to meet the requirements are either justified in the report or carried as open or confirmatory items to be completed and evaluated in supplements to this report before restart. 

The contractor is paid per foot drilled. Whilst this will provide an incentive to make hole quickly, the same risks are involved as in the turnkey contract. Footage contracts are often used for the section above the prospective reservoir where hole conditions are less crucial from an evaluation or production point of view. 

Once the need for a LIMS has been justified, specifications have been written, and available vendors surveyed, the LIMS selection process begins. Each vendor s product must be evaluated for conformance to the specification and flexibiUty for future modifications. If an adequate solution is not available commercially, consideration must be given to constmcting a custom LIMS solution. This could be done in-house or by contractors. If the custom option is preferred, resources needed for future modifications and maintenance should be factored into the cost. 

Usually, the evaluation is done by in-house company personnel, a contractor, or a combination. For capital cost estimation it is difficult to beat a contractor. Fie will, of necessity, have an excellent cost-estimation group that is familiar with a wide range of processes and is on a first-... 

Equally important, the contractor s cost-estimation team will have established a sharply honed, systematic approach including all the correct checklists, forms, and report formats designed to preclude oversights. The team will have developed the discipline, as a result of competitive bidding, to produce the kind of final report that fits in correctly with any type of economic evaluation. 

Side studies should be done by in-house company personnel if possible. If communications are not perfect, it is easy for a contractor to provide a study on the wrong basis. Company design and operating and economic philosophy often are not fully communicated to the contractor with resulting waste of time. In any event, as discussed previously, in-house personnel must do economics. Therefore, use of a contractor is limited to the same portions of the side study as for the overall evaluation. 

Use the LAQ profile and other available information to evaluate the design, operation, and usage of the building, identify potential LAQ problem locations, identify staff and contractors whose activities affect indoor air quality. Review and revise staff responsibilities to ensure that responsibilities that may affect indoor air quality are clearly assigned. 

Contractors at Sites B, D, G, I, and J had incomplete sampling practices and as a result were not able to evaluate PPE levels based on monitoring data. Eor example, both contractors SSAHPs at Site I lacked provisions for monitoring site hazards such as metals, pesticides, herbicides, and semi-volatile organic compounds (SVOCs) that could not be evaluated with a PID. Since worker exposures to the range of hazards on site had not been characterized, PPE was not selected based on its performance relative to the nature and level of site hazards. 

Contractors at Sites C and H had acceptable programs for monitoring the effectiveness of PPE and decontamination procedures. The Site H contractor had established PPE and decontamination programs consistent with HAZWOPER requirements. The Site C contractor developed decontamination procedures that included specifications for periodically evaluating the effectiveness of decontamination methods... 

Establish the criteria for training the contractor/subcontractor. Review the completed contractor/subcontractor prequalification forms and provide an evaluation of the contractor/subcontractor s ability to meet the host organization s health and safety policies and procedures. Contractors/subcontractors must demonstrate their ability to meet established criteria, to the satisfaction of the health and safety professional, in order to be considered for prequalification. 

The regional quality assurance representatives will review for approval the completed contractor/subcontractor prequalification forms and provide an evaluation of the contractor/subcontractor s ability to meet quality assurance standards. 

The performance by the contractor/subcontractor should be noted on an evaluation form and a copy sent to regional purchasing for inclusion in the prequalification file. 

Part evaluation by accredited contractors or test laboratory provides independent verification which can substitute for your own receiving inspection, providing you maintain control over the contractor. 

Quality of installation and the adherence to design specifications of the equipment should be evaluated to ensure that errors during shipping and installation were not made. Often overlooked at this phase are human factors considerations for the construction crew, such as selection of the contractor, training of the crew, lighting, shift work, procedures, and supervision. 

There are two words to watch out for or equivalent. What is called equivalent by a designer or contractor may not be equivalent to the laboratory operator. Sometimes this problem arises when a specified item has been superseded by a new and improved model. However new or improved the new model may be, there could be some reason why it would not be right for a particular application. Any substitution must be viewed as suspect. It is always wise, of course, to listen carefully to all suggestions from building professionals and evaluate the reasons they give for a proposed change, but nothing should go imquestioned. 

The PSM Rule specifies that DOE contractors use one or more of the following methodologies, as appropriate, to determine and evaluate the hazards of the process being analyzed ... 

This subcommittee prepared the broad outline for the book, identified the scope and major key references, and selected the title "Guidelines for Chemical Reactivity Evaluation and Application to Process Design" as representative of the concepts desired. The TNO Prins Maurits Laboratory, Rijswijk, The Netherlands, was chosen as the contractor with Dr. A. Henk Heemskerk as the project manager. 

The committee recognizes and appreciates the extensive support of the Army ACWA team and the committee interactions with stakeholders and the Dialogue group, particularly the four members of the Dialogue known as the Citizens Advisory Technical Team (CATT). Members of the CATT attended all open meetings of the committee and shared information and their views with us. I believe our relationship with the sponsor, PMACWA, and his team and support contractors has been effective and constmctive and that the committee has been given the best available information to conduct this evaluation of the three technologies.
