Training confined space entry

No employee may enter a confined space or serve as an attendant until confined space entry training has been completed and documented. 

Notify employees in the adjacent area of the entry and work to be done. Assign only employees who have had confined space entry orientation or confined space entry training. 

Employees stated that they had received confined space training and were prepared to perform confined space rescue, but they had not rehearsed rescue procedures. In addition, the confined space entry permit form for both contractors did not ask for all required information. For example, the form did not require documentation of the duration for the permit, the intended communications procedures for entry operations, or documentation of hot work performed during confined space entry operations. Completed permits did not contain documentation of hot work performed during confined space entry operations, even though hot work had been performed during such operations at least twice during the project. 

Contractors at Sites E, H, and J had documented confined space programs but had not fully implemented these programs. The Site H contractor had established a permit-required confined space entry program consistent with HAZWOPER requirements however, onsite procedures were not completely consistent with the written program or OSHA requirements. Procedural or operating procedures, for example, operator response to alarms, emergency response procedures, safety rules and standard procedures, training. An example is a confined space entry procedure. 

Safety and loss prevention professionals may also look for potential joint ventures within the public sector (such as municipalities) for various specialty services, such as confined-space injury and rescue. Often the expenditure required to acquire equipment for such specialty areas can be extremely cost prohibitive however, compliance with the OSH A standard must be achieved and maintained. A joint venture with a local fire department that already owns the confined-space equipment may be a viable option, considering the capital expenditure for equipment, cost of training, and potential legal risks. Some safety and loss prevention professionals have been able to enter into joint ventures with municipalities or volunteer fire organizations to have such activities as confined-space entry and rescue performed by these organizations. 

Procedures were rewritten, as many did not exist or were outdated. Critical procedures such as lockout and tag-out and confined space entry were reexamined by subcommittees and rewritten, issued, and the relevant training given. 

Station at least one attendant outside during entry and coordinate actions with contractors used to conduct rescue procedures. Establish a written permit system and review the effectiveness of the system annually. OSHA also requires training to ensure that employees involved in confined space work can perform their job functions safely. This training covers specific items for the authorized entrant, the attendant, and the entry supervisor. Training will cover the following authorized entrant responsibilities (1) hazards involved in confined space entry, (2) use of appropriate PPE for confined space entry, (3) communication policies with the attendant, (4) requirement to leave... 

Training for confined space entry should be geared toward your involvement or likelihood of involvement with confined spaces and hazardous atmospheres. The training could range from how to use space heating devices and carbon monoxide buildup, to full blown entry/exit preparation and execution, air monitoring, respirator use, and rescue.

The permit-required confined spaces standard defines a "confined space " lists specific employee training requirements, equipment, and personal protective equipment required prior to entry and communications and emergency procedures. Appendix A to the 1910 standard also includes an informative Permit-Required Confined Space Decision Flow Chart, as well as an example of a Confined Space Entry Permit. 

In evaluating confined space accidents in the past, certain scenarios seem to continuously occur. These included the failure to recognize an area as a confined space failure to test, evaluate, and monitor for hazardous atmospheres failure to train workers regarding safe entry and failure to establish rescue procedures. At times, either natural or mechanical ventilation is enough to make the space safe, which can be provided prior to confined space entry. Appropriate atmospheric tests must be performed to check for oxygen deficiency, explosive concentrations, and toxic substances in the confined space before entry. The atmosphere inside the confined space mnst be frequently tested or continuously monitored during the conduct of work.

Plant 5 fractionates natural gas liquids (C2+, C3+) streams into products including ethane, propane, hutane, pentane and natural gasohne. If the atmosphere in a confined space is IDLH due to a high concentration of an air contaminant or oxygen deficiency, those who must enter the space to perform work must wear a pressure-demand SCBA or a combination pressiue-demand airline and SCBA that always maintains positive air pressure inside the respiratory inlet covering. Fully trained and equipped rescue must be onsite and ready to respond if needed. This is the best safety practice for confined space entry and is required at the company. 

Note In practice (in the real world of performing confined space entry operations), common routine often designates the entry supervisor as the "competent" or "qualified" person. The competent or qualified person is that entry supervisor who has had the appropriate training and experience and possesses the knowledge required to supervise, and bring about, correct confined space entries. An entry supervisor may also serve as an attendant or as an authorized entrant, as long as that person is trained and equipped as required by the confined space entry standard for each role he or she plays. Also, the duties of entry supervisor may be passed from one qualified individual to another qualified individual during the course of an entry operation.
