Preparation of 3-n-Butyiamino-4-Phenoxy-5-Suifamyibenzoic Acid To a suspension of 3-amino-4-phenoxv-5-sulfamylbenzoic acid (10 grams) in n-butanol (200 ml), concentrated sulfuric acid (2 ml) was added while stirring. The reaction mixture was heated under reflux under conditions in which the water formed during the reaction could be removed. IWhen, after dilution with n-butanol, the N MR-spectrum of a sample of the reaction mix- [Pg.201]

4- phenoxy-5-sulfamylbenzoate formed as an intermediate was more than 90% converted to the corresponding 3-n-butylaminobenzoate, 2 N sodium hydroxide (200 ml) was added and the boiling was continued for 45 minutes. After the saponification, the reaction mixture was neutralized to pH 8 by addition of concentrated hydrochloric acid. [Pg.202]

By cooling, the sodium salt of 3-n-butylamino-4-phenoxy-5-sulfamylbenzoic acid precipitated. It was filtered off and recrystallized from water (100 ml). The sodium salt, crystallizing with 3 molecules of water, was then dissolved in boiling water (200 ml), 1 N hydrochloric acid was added to pH 2.5, and after cooling the precipitated 3-n-butylamino-4-phenoxy- [Pg.202]


Diuretics are needed to return to normal the expanded extracellular volume that other antihypertensive agents produce, such as fluid retention and blood volume expansion, via compensatory mechanisms of the body. The loss of efficacy of antihypertensive agents can be restored if a diuretic is used concomitandy. In the treatment of hypertension, high ceiling or loop diuretics, such as furosemide, ethacrynic acid, and bumetanide, are no more efficacious than the thiazide-type of diuretics. In fact, these agents cause more side effects, such as dehydration, metaboHc alkalosis, etc, and therefore, should not be used except in situations where rapid elimination of duid volume is cleady indicated. [Pg.142]

Potassium Sparing Diuretics. Triamterene and amiloride, potassium sparing diuretics, by themselves produce only slight antihypertensive effects. The main use is to prevent or to treat the hypokalemia induced by thiazide-type and high ceiling loop diuretics, such as furosemide and bumetanide. [Pg.142]

Ototoxicity, as evidenced by transient or permanent hearing loss, is a serious side effect of ethacrynic acid, and occurs less frequently with furosemide. Bumetanide is claimed to have only 20% of the ototoxic potential of furosemide (43). It has been reported that patients treated with torasemide at high doses for four weeks did not suffer hearing loss (36). [Pg.207]
