Tolnaftate (Aftate, Tinactin) is available as a cream, solution, powder, or powder aerosol for application twice daily to infected areas. Recurrences following cessation of therapy are common, and infections of the palms, soles, and nails are usually unresponsive to tolnaftate alone. The powder or powder aerosol may be used chronically following initial treatment in patients susceptible to tinea infections. Tolnaftate is generally well tolerated and rarely causes irritation or allergic contact sensitization.

At the onset, it must be emphasized that although the systemic administration of the antihistamines rarely engenders sensitization, topical applications such as antihistamines not infrequently produce allergic contact sensitivity. Once the patient is sensitized by topical application of the antihistamine, an eczematous contact dermatitis may occur from the antihistamine or from immunochemically related compounds. As a rule the systemic administration of an antihistamine to which there has been topical sensitization will not only reproduce the original allergic eczematous contact dermatitis, but at times a generalized dermatitis will occur with a resulting exfoliative dermatitis (Fisher 1976 b). In some instances, the systemic eczematous contact dermatitis is accompanied by urticarial elements.

As a prerequisite for secondary prevention, the acceptance of skin damage as a normal effect of work has to be counteracted. Early irritant skin damage, e.g., interdigital eczema, is an ideal prompt for efficient secondary prevention because, at this stage, allergic contact sensitization has usually not yet occurred (Schwanitz et al. 1996 Schwanitz and Uter 1998) (Figs. 2 and 3).
