There are hundreds of topical steroid preparations that are available for the treatment of skin diseases. In addition to their aforementioned antiinflammatory effects, topical steroids also exert their effects by vasoconstriction of the capillaries in the superficial dermis and by reduction of cellular mitosis and cell proliferation especially in the basal cell layer of the skin. In addition to the aforementioned systemic side effects, topical steroids can have adverse local effects. Chronic treatment with topical corticosteroids may increase the risk of bacterial and fungal infections. A combination steroid and antibacterial agent can be used to combat this problem. Additional local side effects that can be caused by extended use of topical steroids are epidermal atrophy, acne, glaucoma and cataracts (thus the weakest concentrations should be used in and around the eyes), pigmentation problems, hypertrichosis, alleigic contact dermatitis, perioral dermatitis, and granuloma gjuteale infantum (251).

IL-lRa has been cloned (Arend 1991), and in experimental allergic inflammation of the airways recombinant IL-lRa has some inhibitory effect on alleigic inflammation in animals (Selig and Tocker, 1992 Watson et al., 1993). Studies of human recombinant IL-lRa in asthma are currently in progress. Similar antagonists of other cytokines may be discovered which could lead to the development of future antagonists.

The adverse effects include digestive disturbances, neurological symptoms, and manifestations of alleigic responses. As many as half of the patients taking it are incapacitated by some of these adverse reactions for several hours. Whether these symptoms are caused by hypersensitivity to the drug, the parasite, or by a manifestation of the disease is not known. Overall, effects are dose-related and transient. 

This drug is indicated for infections of skin and nails due to dermatophytes or Candida species and vulvovaginal infections due to Candida species. Miconazole is used orally for prophylactic purposes in patients who have been treated with cytostatics or immunosuppressants, in particular, to prevent candidosis of the mouth and digestive tract. Side effects include possible nausea during oral treatment. Miconazole tablets may increase the anticoagulant effect of coumarin derivatives. Topical treatment with cream or powder is well tolerated. Local irritation and alleigic reactions of the skin and mucosa occur only rarely.
