Severe combined immunodeficiency (SOD) is characterized by impaired B and T lymphocyte function that makes affected persons vuinerabie to iife-threatening infections. 

By promoting the formation of T lymphocytes, thymic factors are used to enhance T-lymphocytic functions. Thymic factors have been used with some success in clinical trials in patients with severe combined immunodeficiency, DiGeorge s or Nezelof s syndrome, and viral disorders. Studies with thymodulin show promise in treating symptoms in asthmatics and patients with allergic rhinitis. The primary consideration in the use of thymic factors for immunodeficiency states is the presence of T-lymphocyte precursors.

Several age-related changes contribute to the high incidence of infections in geriatric patients. There appears to be a reduction in host defenses in the elderly, manifested in the increase in both serious infections and cancer. This may reflect an alteration in T-lymphocyte function. In the lungs, a major age and tobacco-dependent decrease in mucociliary clearance significantly increases susceptibility to infection. In the urinary tract, the incidence of serious infection is greatly increased by urinary retention and catheterization in men.

One boy who had been immunized shortly after birth developed ipsilateral axillary lymphoma at the age of 6 months. Microscopically the picture was typical of BCG histiocytosis. The child survived. In a study of mistletoe on T lymphocytes in patients with prostate, breast, or colorectal cancer, a decrease in T lymphocyte function was observed in colorectal and prostate cancer patients on the "swift" dose escalation scheme. 

Future studies are needed to determine the CMV gene products mediating these processes and the precise molecular mechanisms for these effects. These gene products will undoubtedly be unique tools for studying JAK/STAT signal transduction, MHC expression and antigen presentation, T lymphocyte function, and tissue transplantation. 

In 1975, Giblett and co-workers (18) described a deficiency of the enzyme purine nucleoside phosphorylase in patients with severe combined immunodeficiency disease characterized clinically by a severe abnormality of T-lymphocyte function. ile these patients often exhibit hypouricemia as a result of the deficiency of purine nucleoside phosphorylase, they exhibit at the same time accelerated levels of purine biosynthesis de novo with increased excretion of adenosine, deoxyinosine,guanosine, and deoxyguanosine (19). In addition, the intracellular levels of PRPP are elevated in patients with purine nucleoside phosphorylase deficiency. It is assumed, therefore, that the accelerated rate of purine biosynthesis may be due, at least in part, to the elevated levels of PRPP. It has been suggested that a decreased availability of the substrates hypoxanthine and guanine leads to a decreased functional activity of the enzyme HPRT thus leading to decreased consumption of PRPP and hence the elevated levels observed. 

Tetramisole is an antihelmentic which acts on the cyclic nucleotide phosphodiesterases. It actually consists as a combination of optical isomers, the most active one being levamisole. Levamisole was the first synthetic chemical that exhibited immunomodulatory properties. It appears to restore normal macrophase and T-lymphocyte functions.
