Systemic Chronic Candidiasis

Side effects of inhaled corticosteroids are relatively mild and include hoarseness, sore throat, oral candidiasis, and skin bruising. Severe side effects such as adrenal suppression, osteoporosis, and cataract formation are reported less frequently than with systemic corticosteroids, but clinicians should monitor patients receiving high-dose chronic inhaled therapy. 

Use only to treat severe systemic fungal disease Coccidioidomycosis, candidiasis, cryptococcosis, pseudoallescheriosis (petriellidiosis, allescheriosis), paracoccidioidomycosis, and for the treatment of chronic mucocutaneous candidiasis.

Younger than 2 years of age) - Daily dosage has not been established. Minimum treatment is 1 or 2 weeks for candidiasis and 6 months for the other indicated systemic mycoses. Chronic mucocutaneous candidiasis usually requires maintenance therapy.

Ketoconazole was the first imidazole derivative used for oral treatment of systemic mycoses. Patients with chronic mucocutaneous candidiasis respond well to a once-daily dose of 200 mg of ketoconazole, with a median clearing time of 16 weeks. Most patients require long-term maintenance therapy. Variable results have been reported in treatment of chromomycosis. 

Humans with deficient IFNy response show several immunological defects, such as increased susceptibility to infections with intracellular pathogens. Impaired IFNy production has been demonstrated in human pathological conditions, namely chronic mucocutaneous candidiasis and systemic lupus erythematosus. ... 

Chronic Systemic Candidiasis 250 ml of 0.15 percent once a week for 10 treatments and then monthly follow-up for 10 months. Comment Clinical response not observed until after the-fourth treatment, then gradual improvement continued. Maintained on monthly treatments. 

Hepatosplenic candidiasis—Clinical presentation often manifested only as fever while a patient remains neutropenic (<1000 WBC/ mm ). When the WBC count increases to >1000 cells/mm, imaging studies can detect the presence of abscess or microabscesses in the liver and spleen, often found with acute suppurative and granulomatous reactions. Infection may persist for months and ultimately cause the patient s death despite aggressive systemic therapy with antifungal agents. Also known as chronic systemic candidiasis.
